Case Manager, Community Outreach
Department: Older Adult Services – Home-Based Services
Compensation: $30.22 – $34.07 hourly
Hours & Schedule: Full-time, Year-to-Year Contract Renewal
Position Overview
Queens Community House (QCH) Older Adult Services Department is seeking a Case Manager/Community Outreach Worker to join our Home-Based Services division. In this role, you will become a vital part of helping older adults live safely and independently in their homes.
This is a dynamic, person-centered position that combines compassionate case management with proactive outreach and community engagement. We are looking for someone organized, empathic, and outgoing—someone who thrives on building relationships and promoting services that enhance the quality of life for older New Yorkers.
Your Impact
Assessment & Care Planning
Conduct intake interviews and in-home assessments to examine client strengths, challenges, and needs.
Create and update individualized care plans that reflect each client’s cultural background and goals.
Provide accurate information on services, benefits, and entitlements.
Coordinate client care with QCH programs, partner organizations, and other professionals.
Maintain complete and accurate case records and reassess eligibility regularly.
Build trust through cultural attunement, active listening, and clear communication.
Outreach & Engagement
Conduct home visits to current and potential clients to increase program participation.
Collaborate with community partners to promote services and resources.
Deliver presentations about the program to community groups and stakeholders.
Actively represent QCH with an approachable, professional demeanor while promoting the program’s benefits and impact.
What We’re Looking For
A people-centered professional with strong communication and relationship-building skills.
Empathy, organization, and attention to detail.
Experience with case management, outreach, or community-based services is a plus.
Comfort working independently and collaboratively in the field.
