Job Summary

The CHW - Patient Navigator will act as part of the patient care team by implementing clinic-based programs that address patient needs related to social determinants of health, as well as patient in-reach initiatives and campaigns. This role will learn and obtain knowledge around the resources and culture of the local community to better understand the needs of patients and have a positive impact on the overall patient experience and health outcomes.


Education - Associate’s degree. Experience in a health care setting may be substituted year for year for the education requirement.
Experience - One year experience in a health care setting, community health education and/or outreach position.
Other Requirement(s) - Proficiency in Vietnamese, Cantonese, Mandarin, Spanish, Somali, or other languages strongly preferred. Proficient computer skills. 


8:30am to 5:00pm Monday through Friday
