What You Will Do

  • In conjunction with interdisciplinary team members, develops and implements goal-focused treatment plans for clients and their families. Reviews the plan and outcome with clients initially and as plan changes.
  • Under direction of supervisor, facilitates regularly scheduled site-based education, skills training, and support activities.
  • Coordinates and collaborates with other services on the treatment plans, and client’s needs, as well as follow up, on linkages made between clients and other service providers to ensure continuity of care.
  • Conducts regularly scheduled home visits to provide skills teaching, support, mentoring, and advocacy; frequency of visits to be determined by treatment plan developed with Program Coordinator/Manager.
  • Participates in interdisciplinary team meetings and case presentations.
  • Assists Program Coordinator/Manager in the achievement of program goals.
  • Attends in-service education programs and other training, as required by the Agency and funding sources.
  • Maintains proper documentation and record of statistics in accordance with the Program's record keeping procedures.
  • Participates in special projects and performs other duties as assigned.
Qualifications:Education:
Bachelor's Degree in social work, health and human services or a related field OR two or more years of equivalent and relevant experience in a social service agency OR an equivalent combination of the education and experience Required

Work Experience:
Minimum of one year of social work experience in a mental health or social service setting with emphasis on family support services and/or family psycho-education PreferredBilingual skills may be required as determined by operational needs.

Pay Range:USD $16.50 - USD $18.03 /Hr. Apply Now
