Join Our Team as a Transition Care Coordinator!
Are you passionate about helping members achieve increased independence and quality of life through community-based services and supports? LTSS is seeking a dedicated Transition Care Coordinator to join our team. The Transition Care Coordinator is responsible for the proactive approach in identifying Choices and ECF Choices members who are potential candidates for transition to the community.
Key Responsibilities:
- Empower members by supporting their health and well-being.
- Collaborate with members Care/Support Coordinator.
- Actively guide all transitions from one setting to another for any individual with challenging behavioral support needs, mental health challenges, or significant medical needs.
- Ensure that all individuals transitioning from one setting to another have access to the wrap-around supports they need
- Ensure comprehensive, holistic, person-centered approach to care during transition process and post facility discharge.
- Facilitate all CLS placements for ECF and CHOICES members, including those involving transitions out of institutional settings.
- Act as a resource and Subject Matter Expert on transitions to the community and Community Living Supports (CLS)
- Develop and maintain relationships with community partners including Community Living Support providers, Tenncare, DDA, DCS, APS and other community organizations.
- This role supports the Middle TN area (Must live in Middle or East TN)
- Available for an 8:00am – 5:00pm CST(no on call) schedule.
- This position is fully remote.
You will be a great match for this role if you have strong case management/behavioral health experience, communication, and organizational skills, as well as attention to detail and ability to create efficient, consistent, and accountable habits. Preferred candidates will have a LCSW, LMSW, or LAPSW license.
Job Responsibilities
- Collaborate with Care/Support Coordinators, educate members and their families/representatives about transition supports and services through family meetings or other outreach opportunities. When necessary, review the transition plan and documents for accuracy and thoroughness in meeting member needs, identifying all the services necessary to safely transition the member to the community. This includes but is not limited to member needs related to housing, transportation, availability of caregiver.
- Assist the assigned Care/Support Coordinator in processes to collaborate with members primary care provider, specialists, and other providers and care programs to ensure comprehensive, holistic, person-centered approach to care post facility discharge.
- Support management with report data, participation in various committees and conference calls related to transitions.
- Develop and maintain relationships with key resources, including TennCare, housing providers, landlords, community service organizations, and faith based initiatives
- 100 % day travel required
- Various immunizations and/or associated medical tests may be required for this position.
Job Qualifications
Experience
- 3 years - Clinical experience required
- 2 years - Experience providing care coordination to persons receiving long-term care and/or home and community based services required
SkillsCertifications
- Proven analytical and problem solving skills and ability to perform non-routine analytical tasks
- Must be able to work in an independent and creative manner.
- Effective time management skills
- Excellent oral and written communication skills
- Strong interpersonal and organizational skills
License
- Valid Driver's License
- Registered nurse with an active license in the state of Tennessee or hold a license in the state of their residence if the state is participating in the Nurse Licensure Compact Act; or Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW); or 2 years experience as an ECF Support Coordinator.
