The Palliative Care Program Coordinator acts as a liaison with our contracted services group for the Palliative and Hospice Care services and associated programs. The coordinator is responsible for supporting clinical staff and physicians with the management of the palliative patient population through the continuum, with particular emphasis on coordination of the patient’s goals of care for those with serious illness. The coordinator assists in identifying patients who may be clinically appropriate for palliative or hospice care and works to facilitate timely referrals and coordination of services in alignment with the patient's goals and needs. The coordinator assists with program process improvements, patient and family programs, managing the accreditation process, and ensuring continuous compliance with regulatory bodies. Responsibilities also include oversight of applicable policies and procedures, staff education, data collection, quality improvement efforts, and chart data abstraction. The coordinator monitors that timely consults are conducted, appropriate orders are written, and nursing documentation is complete and accurate. Qualifications:
Graduate of an approved nursing program with a BSN required.
Current Indiana or compact state RN license required.
Three (3) years of palliative care nursing experience required.
BLS for Healthcare Provider required.
Master’s degree in nursing preferred.
Experience as a Charge Nurse or Supervisor preferred.
