Job Title:
Director of Social Services
Job Summary:
This position requires a functional knowledge of the performance skills required of a Social worker in helping patients with social, emotional and related difficulties associated with their medical condition or recovery and adjustment to said condition. This work is performed as an integral part of a multidisciplinary team and requires that level of commitment and approach. The Social Work Director also supervises and coaches qualified social worker(s) serving in the department. Independence of action and the ability to problem solve and clearly communicate orally and in writing are key. Functional knowledge of a PC, EHR documentation and LTC systems and processes (Care Planning, aide in adjustment, discharge planning, etc.) are essential. Professionalism, compassion, empathy and fortitude a must. Employees in this class are expected to confer with professionals or agencies in or outside of the facility to obtain assistance and guidance for patients. Will also serve as consultant or provide Guidance to the ACF when required. Does other related duties as assigned. This individual reports to the ECF Administrator.
Key Responsibilities:
- Provides social work services to help patients adjust to their illnesses or disabilities; aide in initial adjustment to the facility as well as aiding in the continual accommodation to the environment.
- Supervises and directs Social Worker and Social Work Assistant.
- Develops appropriate Policies and Procedures as related to the Social Work Department.
- Periodically responds and confers with the facility Administrator related to the operations of the department.
- Maintains a monthly report for submission related to services provided to administration.
- Assist in creation of the Comprehensive Care Plan. Create documentation which supports the Care Plan and its changes.
- Assists in the Admission process. Possesses ability to explain admission process to resident and family members.
- Collects and maintains appropriate data from various sources such as professional staff and patients’ families to establish an overall approach to solution of medical and social problems.
- Maintains information on community and health resources which can be utilized during the patients’ care and subsequent to his discharge.
- Records social information to develop patient case histories.
- Participates in the development and implementation of and adherence to the rights and responsibilities of patients.
- Participates in the process of transferring patients to other levels of medical care as appropriate.
- Works with private, voluntary and other public agencies to identify the availability of medical and health related community services.
- Significantly assists each resident to meet a variety of social, psychological, physical, environmental, family, financial and related needs via coordination with the facility Multi-disciplinary team and outside resources.
- Incumbent may provide care/treatment/services to adults and geriatric patients as assigned.
- Acts in the capacity of the “Grievance Officer” to track and monitor grievances and their the solutions filed by residents and families.
- Participates in the Ambassador Program to orient new admissions to the facility and ensure their needs are being met to the best of the facility’s ability.
Qualifications
Qualifications:
Education: Graduation from a regionally accredited or New York State registered four year college or university with a bachelor’s degree in social work, sociology, psychology, human services or resources; MSW preferred.
Experience: Experience with the Geriatric population a plus.
Licensure: Licensed in the State of NY
