As one of the first points of contact for potential participants, the Enrollment & Intake Therapist introduces individuals to Concordance’s life-changing services, guiding them through the enrollment process. Using standardized assessments, you’ll identify participant needs, diagnose, and set treatment goals. In this role, you’ll build a foundational relationship between Concordance and participants and maintain engagement during the transition to post-release programming.
Key Responsibilities - your role will involve:
- Interviewing potential participants prior to release from prison
- Assessing participants’ eligibility for Concordance and obtaining consent from participants for participation
- Serving as a primary point of contact for incarcerated individuals, while maintaining a positive and professional presence
- Administering assessments to inform diagnoses
- Diagnosing participants and creating treatment plans aligned with their goals
- Welcoming newly released participants to post-release programming and reaching out to participants who have not yet arrived at the Center
- Maintaining and updating records in electronic record system
- When working within a correctional center, building strong, professional, positive relationships with prison staff and administration
- Serving as the liaison between participants and post-release care team upon release
- Participating in internal meetings (Behavioral Health and Wellness team meetings, All Team meetings, 1:1 supervisor meeting(s), and training sessions)

- Experience using DSM-5 and assessments to form a diagnosis and set goals and objectives for treatment
- Commitment to professionalism, integrity, and compassion in all interactions
- Master’s degree in counseling, social work, behavioral health, or related field
- Independent clinical license
- 3-5 years of experience working in mental health and substance use disorders, with equal focus on mental health and substance use treatment
- Familiarity with and endorsement of 12 step program
- Ability to pass DOC security clearance
- Driver’s license
- CPR/Adult First Aid certification preferred
- Sit, stand, and walk for a considerable amount of time throughout the day.
- Ability to travel to institutions and facilitate overview sessions and 1:1 enrollment meetings.
