Position Overview:

 The CARE EHE Case Manager will be responsible for outreach and recruitment working with former CARE individuals and families, unengaged new clients and providing services to current CARE clients in areas where the current program has limited resources. They will work in the community, identifying populations to work with and with current case managers to review their cases and identifying appropriate referrals, resources and services. The ultimate goal is to help these clients connect officially with the CARE program, seek immediate resources, and improve their overall health regardless of whether they are part of the CARE program.

This is a challenging and rewarding role that requires a high level of professionalism, empathy, and attention to detail. In this role, they will use their strong communication and interpersonal skills to build relationships with clients and collaborate with other professionals. They will also need to be proficient in using technology and software to maintain accurate case records and documentation. Finally, their knowledge of community resources and services will be critical in connecting clients with the support they need to achieve their goals.

Essential Duties & Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized care plans
  • Coordinate and monitor the delivery of services and resources to clients
  • Identify community outreach opportunities; work with local providers, healthcare organizations, social service agencies to identify appropriate referrals
  • Maintain accurate and up-to-date case records and documentation
  • Collaborate with other professionals and agencies to ensure clients receive the best possible care
  • Work with CARE staff to identify current clients who could benefit from this additional service 

Required Skills and Qualifications:

  • Bachelor's degree in social work, psychology, or related field; master's degree preferred
  • 2+ years of experience in case management or related field
  • Strong communication, organizational, and problem-solving skills
  • Ability to work independently and as part of a team
  • Knowledge of community resources and services
  • Experience working with diverse populations and/or in a non-profit setting
  • Bilingual in English and Spanish
  • Certification in case management or related field

 Physical Requirements:   

  • Prolonged periods of sitting at a desk and working on a computer 
  • Must be able to lift up to 15 pounds at times

 Compensation:

A salary within the range of $24.51 to $33.17 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
