About the Role
Delta Health is seeking a compassionate, experienced, and proactive RN Patient Navigator to join our Emergency Department team. This role is ideal for a Registered Nurse who thrives on collaboration and care coordination and is passionate about providing patient- and family-centered care. As an RN Patient Navigator, you will guide patients through the continuum of care in the ED, serving as a critical link between patients, families, and the interdisciplinary care team.
Key Responsibilities:
- Coordinate and advocate for patient care across departments and services.
- Collaborate with ED care teams and ancillary services (e.g., cardiopulmonary, infusion center).
- Perform medication reconciliation and assist with discharge planning and follow-up care.
- Support the ED team in preventing unnecessary admissions and facilitating appropriate care transitions.
- Provide education and emotional support to patients and families regarding diagnoses, treatments, and community resources.
- Assist patients with referrals to hospice, home health, palliative care, and other support services.
- Participate in staff education, program evaluation, policy updates, and quality improvement initiatives.
- Maintain up-to-date knowledge of community resources and services for optimal patient outcomes.
Qualifications:
- Required:
- Active Colorado RN license or compact state license
- BLS certification (current upon hire)
- ACLS, TNCC, and PALS/ENPC certifications (required within 6 months of hire)
- Preferred:
- Minimum 2 years of Emergency or Critical Care experience
- Case Management experience
- Meditech experience
- Skills:
- Excellent communication and customer service
- Strong analytical and problem-solving ability
- Effective multitasking and team collaboration skills
Physical Demands:
- Must be able to lift and move patients/equipment (40–80 lbs unassisted; up to 250 lbs with assistance)
- Extended periods of walking and standing
- Dexterity for handling medical equipment and documenting care
- Requires auditory, visual, and tactile skills for safe patient care delivery
- Ability to remain effective in high-stress, dynamic situations
