Assist coordination of the outpatient Cardiac Rehabilitation Program while caring for patients with heart disease utilizing established Cardiac Rehab practice standards subscribed by the American Association of Cardiovascular and Pulmonary Rehabilitation.
Essential Duties & Responsibilities: |
- Collects of current and past medical history, risk factor identification, medications, nursing assessment with nutritional as well as psychosocial evaluation. Patient given overview of what to expect and guidelines of cardiac rehab. Given first level of education of needed changes to life style.
- Collects referrals from the physicians practice, verifying correct and acceptable data. Verify what insurance is available and coverage.
- Completes patient assessment; initial complete nursing assessment as well as daily updates with vital signs obtained and recorded. Daily cardiac telemetry with identification of underlying rhythm as well as any anomalies for all phase II patients.
- Performs daily education and documents related to their heart disease at many different layers from anatomy to what weather to avoid.
- Provides individualized treatment plan to exercise and advance intensity. Plans originate from first conversation with patient to re-evaluation every visit.
- Completes a daily report for each patient evaluating their session. Generates Phase II patients’ monthly report evaluating their progress, vital signs, education given, any problems, overall attitude etc. Generates Phase III patients’ reports before their annual or semiannual physician appointments. All monthly, discharge and Phase III reports sent to the patient’s physicians.
- Completes education session with every Phase II patient that completes program to review important topics, reconcile medications and instruct on safely exercising at home.
- Constantly observes patients for any need to consult specialty services. Arranges for all patients to attend dietary education and provide navigation for those who may need assistance for psychological issues, smoking cessation, social services etc.
- Maintains knowledge and skills in cardiac rehab nursing.
- Works with Medical Advisor and Director of Cardiopulmonary services to establish and maintain policies and procedures for the Cardiac Rehabilitation Department
- Assesses and maintains all equipment in working order.
- Maintains current patient logs.
- Ensures accurate and timely billing of cardiac rehab patients.
- Communicates with referring health care organization to facilitate return of local patients to cardiac rehabilitation.
- Patient insurance review, registration, billing and discharge; insurance review=minimum 15 minutes each admission, registration=minimum 30 minutes each admission, billing=30minutes every M,W,F, discharge=30 minutes minimum each patient
- New patient orientation; Initial phone call that includes insurance review, program description and any immediate issues such as health complications, time availability etc. (15-45 minutes avg.) May have the need for follow up phone calls to patient to discuss complications discovered in first call. Orientation first visit 2+ hours.
- Patient correspondence; contacting patients that are ill, injured or had some complication. May be a phone call, inpatient, ER or home visit.
- Patient counseling; counseling starts with initial phone call to schedule patient and in certain circumstances may continue with each visit.
- Reports to work on time and as scheduled; Shows respect for co-workers by following established break times; Keeps personal conversations and telephone calls to a minimum.
- Assists director with researching, developing, writing, and updating policies, procedures, methods, and guidelines; communicates and enforces hospital culture, values and standards of performance in a consistent and thorough fashion.
- Evidences the knowledge and ability to assess and interpret data about the patient’s growth and development in order to identify each patients needs and provide the appropriate care, including age-related care, where appropriate.
- Complies with federal, state, and local legal requirements by studying existing and new legislation; anticipating legislation; enforcing adherence to requirements; advising management on needed actions; ensures compliance with Joint Commission and other external and internal regulatory agencies.
- Maintains the integrity of confidential patient information in accordance with HIPAA and hospital policy, including accessing, using and disclosing patient information relevant to the individual employee’s job duties. Also maintains confidentiality of other customer and hospital business affairs.
- Adheres to the standards of Magruder Hospital’s corporate compliance policies and seeks guidance and training when necessary to resolve questions about the policies. Reports violations in accordance with corporate compliance and organizational standards.

- Travel is primarily local during the business day, although some out-of-area and overnight travel may be expected.
