We’re Hiring: Clinical Case Manager – Family Preservation Team (Manchester, NH) 

At Waypoint, we believe in keeping families safe, strong, and together. Our Family Preservation Team is seeking Clinical Case Managers who are compassionate, adaptable, and committed to helping children and families achieve permanency and well-being.

About the Role
As a Clinical Case Manager, you’ll provide trauma-informed, strength-based interventions and case management for children and families referred by child welfare agencies. Working across homes, schools, child protection, and courts, you will:

  • Provide individual and family clinical support to children, parents, foster families, and birth families

  • Develop treatment plans and utilize solution-focused casework to build safe, healthy family systems

  • Deliver consistent, face-to-face support to high-need families

  • Collaborate with multiple systems and advocate for permanency and stability

Minimum Qualifications

  • Master’s degree in social work, counseling, or marriage & family therapy with 2+ years’ direct experience, OR

  • Bachelor’s degree in the same fields with 5+ years’ direct experience

  • Knowledge of DCYF and in-home visiting experience strongly desired

  • Willingness to train in evidence-based practices

  • Strong documentation, record-keeping, and treatment planning skills

  • Valid driver’s license, reliable transportation, and insurance required

  • Must pass criminal background check (no history of abuse/neglect/exploitation)

  • Ability to manage complex cases and travel throughout NH for home visits

  • Strong collaboration, flexibility, and communication skills

  • Availability for after-hours on-call rotation support
