The Special Immunology Services (SIS) Clinic provides tertiary care for outpatients with HIV/AIDS. A multidisciplinary team provides culturally relevant and respectful healthcare and medical and social services for clients with HIV. Responsibilities include: completion of psychosocial assessments, education and support, case management, community advocacy, referrals to community agencies and consultant liaison with acute inpatient units.
A Social Worker in the Special Immunology Services (SIS) Clinic provides evidence-based support for patients infected with HIV and those co-infected with Hepatitis C. Through in depth psychosocial assessment, which includes the collaboration of the multidisciplinary team, the social worker will identify patient strengths and supports, and will counsel and implement plans for intervention for the patient living chronically with HIV. The Social Worker provides education related to HIV, sexuality and resources in the community. In some cases, the Social Worker will be aware of evidence-based practice in the field of HIV and co-infected Hepatitis C and will integrate research and practice.

1. Master's Degree in Clinical Social Work from an accredited University
2. Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
3. Minimum 3-5 years recent clinical experience providing support and individual, marital, family and group counseling to individuals with HIV and/or Hep C
4. Demonstrated skills in the performance of a therapeutic role including strong assessment skills, psychosocial intervention, counseling, case management, and advocacy skills
5. Demonstrated knowledge and skill in suicidal risk assessment and crisis intervention
6. Working knowledge of the Mental Health Act, Consent to Treatment Act, Privacy Legislation, and Health Protection and Promotion Act
7. Willingness to work with a growing program and actively evaluate and provide feedback to the program as a whole
8. Demonstrated ability to work within an interdisciplinary team in a fast paced, changing environment
9. Strong skill set in mental health, behavior modification, substance abuse, women's health issues, cultural diversity, immigration, working with and supporting the homeless/outreach work/sex trade workers, HIV and or Hep C
10. Proven ability to organize, problem-solve effectively, and deal with complex individual/family situations that can escalate quickly
11. Demonstrated commitment to ongoing learning and teamwork
12. Demonstrated ability to provide education to clients, health care providers, students and the community, to inform and encourage healthy lifestyle changes, to assist in demystifying AIDS and supporting clients when dealing with issues of stigma in relation to HIV and AIDS
13. Demonstrated interest in teaching social work students at the graduate and undergraduate level
14. Strong connections with a wide variety of community organizations is considered a definite asset
