Position Summary:
We are seeking a dedicated and empathetic Hospice Nurse to join our team. This full time position focuses on delivering exceptional end-of-life care by addressing physical, emotional, and spiritual needs of hospice patients and families. The ideal candidate will collaborate with an interdisciplinary care team to create personalized care plans, provide education, and uphold the highest standards of hospice care. Key Responsibilities:
Patient Care
• Conduct comprehensive nursing assessments to evaluate patient and family needs.
• Develop and implement individualized care plans in coordination with patients, families, and the care team.
• Administer medications, monitor vital signs, and manage symptoms to ensure patient comfort and dignity.
• Educate patients and families on self-care techniques and the hospice philosophy.
• Advocate for patient and family needs to deliver personalized and compassionate care.
Communication
• Actively participate in interdisciplinary team meetings to share updates and recommendations.
• Maintain accurate clinical documentation, including changes in patient conditions.
• Facilitate clear communication between patients, families, and the care team.
Additional Duties
• Serve as a Case Manager when assigned, coordinating care for an assigned caseload.
• Supervise ancillary personnel and delegate responsibilities as necessary.
• Participate in on-call and holiday duties as required.
• Engage in ongoing professional development and quality improvement initiatives.
Qualifications:
• Acute care and hospice experience is advantageous.
• Current RN licensure in the state and CPR certification.
• Valid driver’s license with reliable transportation and required insurance coverage.
• Demonstrated knowledge of hospice care principles and symptom management techniques.
• Strong communication, problem-solving, and interpersonal skills.
• Ability to lift and transfer patients and handle equipment as needed.

Pay Grade: 22
Range: $33.00 - $49.25
Starting pay will be determined based on experience and relevant credentials
