Job Title: Hepatitis C Patient Navigator (HCVPN) 

Department: Community Prevention Services – Programs and Services  

Position type: Full Time 37.5 Hours  

FLSA: Non-Exempt 

Job Summary: 

The Hepatitis C Patient Navigator (HCVPN) plays a vital role in helping patients living with or at risk of Hepatitis C connect with care, stay engaged in treatment, and achieve better health outcomes. This position combines patient advocacy, education, and clinical support, including phlebotomy and rapid testing, to ensure individuals receive the care and resources they need. Ideal candidates are compassionate, adaptable, and committed to harm reduction and inclusive community health. 

Duties/Responsibilities: 

  • Serve as a primary advocate and contact for Hepatitis C patients, offering education, support, and care navigation.
  • Link newly diagnosed individuals to care, provide treatment adherence education, and support retention throughout the care process.
  • Schedule and coordinate patient appointments, assist with insurance and medication programs, and collaborate with interdisciplinary teams.
  • Perform phlebotomy, provide rapid HCV/HIV/STI testing, and document testing services accurately.
  • Conduct community outreach and educational programming to raise awareness and strengthen partnerships.
  • Maintain documentation and compliance standards while supporting diversity and inclusion.

Required Skills/Abilities:  

  • Strong knowledge of harm reduction, trauma-informed care, and motivational interviewing.
  • Excellent communication and interpersonal skills to engage diverse populations.
  • Ability to work collaboratively with both patients and community partners.
  • Proficiency in phlebotomy and testing procedures.
  • Commitment to inclusivity and culturally competent care.

Education and Experience: 

  • Bachelor’s degree in Public Health, Education, Social Work, or a related field required.
  • Experience working with individuals who use substances and/or in community health preferred.

 

Physical Requirements:  

While performing the duties of this job the employee is required to stand, sit, walk, use hands to finger, handle, or feel; reach with hands and arms, talk and hear. Occasionally the employee must stoop, bend and lift or move up to 25 lbs. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
