LOCATION:  

Napa and Solano Counties; this position will sit at West Fairfield 

PAY RANGE:  

$26.16 to $31.98/hr (depending on experience)  

SCHEDULE:

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY/OVERVIEW: The Peer Support Advocate (MAT) reports directly to the Director of Substance Use and Latinx Services and aims to prevent relapse, encourage engagement of people who use substances and struggle with addiction. They will empower clients through strengths-based coaching supporting linkages to community resources and educating beneficiaries and their families

MINIMUM POSITION REQUIREMENTS: EDUCATION, EXPERIENCE, SKILLS/TRAINING  

  1. Education: 

High school diploma or equivalent required 

  1. Experience/Lived Experience: 

The Peer Advocate (MAT) uses personal/lived experience (identifying as a person in recovery from substance use/addiction to provide support to individuals struggling with addiction. 

  1. Special Skills/Training: 
  • Cal MHSA Peer Support Training and/or Certification (preferred).
  • Bilingual Spanish/English preferred.
  • BLS Certification required. 

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES  

1. 

Assist clients with problematic substance use and mental health challenges by offering a shared understanding, respect, and encouragement that includes sharing lived experiences providing support and advocacy, and hope in a non-judgmental way.  

2. 

Assist in practical supports by promoting wellness and recovery, assisting in setting recovery goals, action steps and achievement of goals. Assist people in preparing for health care/treatment visits and appointments and managing complex health systems. 

3. 

Serve as a role model, coach, connector and resource navigator 

4. 

Collaborate with programs to assist in the appropriate transfer of care and follow-up services. Supporting linkages for transitions of care to empower participants including family members. 

5. 

Offer informational support by assisting in daily management of tasks, such as attending appointments, counseling sessions including scheduling appointments, assisting with referrals, discharge planning, and referrals 

6. 

Offer emotional support, encouragement and coaching to help clients build self-confidence and stability. Assist in practical supports by promoting wellness and recovery by assisting in setting recovery goals, action steps and achievement of goals 

7. 

Support with reducing barriers to success by connecting with other people in recovery and getting involved in substance free activities. 

8. 

Assist with coordination of care on Team, by offering insights into client progress and the development of the program. 

9. 

Assist in problem solving to reduce crises. 

10. 

Develop approved plans of care for each individual 

11. 

Provide culturally competent individual and group services promoting recovery, resiliency, engagement, socialization, self-sufficiency, self-advocacy and the developments of natural supports. 

12. 

Complete necessary documentation and tracking in a timely manner 

13. 

Responsible for remaining within established ethical standards, including those of confidentiality and mandated reporting 

14. 

As safe and appropriate to do so, transport clients or support with transportation arrangements 

15. 

Observes all health center and departmental policies governing conduct while at work (e.g., telephone and computer use, electronic messaging, smoking regulations, parking, breaks and other related policies) 

16. 

Attend training/conferences as arranged 

17. 

Performs other duties as assigned.
