Hospice Clinical Manager

Department: Hospice
Reports to: Director of Hospice
Job Type: Full-time, Salaried

Position Summary

The Hospice Clinical Manager oversees coordination and delivery of patient care within the hospice program, ensuring services meet the highest standards of quality and compliance.

As a key member of the leadership team, this role works closely with the Hospice Director to review clinical appropriateness for new and current patients, support staff in the field, and manage day-to-day operations. This position is critical in fulfilling Healthkeeperz’s mission: Caring for all people for the glory of God.

Core Responsibilities

Patient Care & Oversight

  • Manage intake, evaluation, and case assignments.
  • Conduct case reviews and home visits to ensure quality care.
  • Monitor patient progress and adjust care plans as needed.

Clinical Supervision & Team Development

  • Supervise clinical staff, providing guidance and performance feedback.
  • Lead interdisciplinary team meetings and case conferences.
  • Mentor, train, and develop clinical team members.

Quality & Compliance

  • Ensure care delivery aligns with hospice philosophy, state/federal regulations, and organizational policy.
  • Lead quality improvement initiatives and participate in audits and reviews.

Operational Management

  • Oversee scheduling, resource allocation, and administrative functions.
  • Participate in on-call rotation for urgent consultations.
  • Support budgeting and operational effectiveness of the hospice program.

Documentation & Reporting

  • Ensure timely and accurate medical documentation.
  • Prepare reports as required for regulatory and organizational purposes.

Qualifications

  • Education: RN license required; BSN preferred (ADN minimum).
  • Experience: Minimum two years of supervisory or management experience in hospice, home health, or community health within the past five years.
  • Licensure: Active RN license in North Carolina.
  • Certifications: Current CPR certification.
  • Skills: Strong leadership, communication, and organizational skills. Proficient in nursing judgment, care planning, and principles of pain/symptom management.

Work Environment

  • Frequent travel to patient homes.
  • Occasional lifting and extended periods of standing or sitting required.
  • Participation in administrative on-call rotation.

Requirements

Qualifications:

  • Education: Registered Nurse with a BSN preferred; minimum requirement of an ADN.
  • Licensure: Current RN licensure to practice in North Carolina.
