Park Nicollet is looking to hire an MTM Pharmacist to join our team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
The purpose of this position is to improve population based and patient specific medication related outcomes through collaboration with a multi-disciplinary medical team in the ambulatory setting.
The pharmacist assumes a primary responsibility and accountability for the provision of safe and effective drug therapy and other medication management services intended to achieve outcomes related to the mitigation or prevention of disease, reduction of patients’ symptoms, prevention of adverse drug events and improvement of patients’ quality of life.
Work Schedule:
Monday - Friday 8:30 AM - 5:00 PM
Clinic locations: St. Louis Park and St. Paul
2 or 3 days in-person depending on clinic needs.
2 or 3 days remote.
Required Qualifications:
- Education, Experience or Equivalent Combination:
- Candidate must have graduated from an accredited college of pharmacy on or after May 1996, or completed a structured and comprehensive education program approved by the Accreditation Council of Pharmacy Education (ACPE) and the Board of Pharmacy with didactic and clinical elements upon hire.
- Candidate must have two years of recent clinical pharmacy experience in ambulatory care as part of a multidisciplinary team within the last five years, or successful completion of one year specialized post-doctoral residency.
- Licensure/ Registration/ Certification:
- Current Minnesota Pharmacist license and maintain that license in good standing
- Wisconsin Pharmacist license must be obtained within 6 months of start date and maintained in good standing.
- Knowledge, Skills, and Abilities:
- The desired candidate would have deep knowledge of evidence based clinical guidelines, pharmacology, pharmacodynamics, and pharmacokinetics, pathophysiology and therapeutic disease states commonly found in ambulatory patients.
- Personal computer utilizing spreadsheets, word processing, and email. Electronic medical record system, MTM billing software, phone, fax, and pager.
Preferred Qualifications:
- (BCACP) or Board Certified Pharmacotherapy Specialist (BCPS) preferred.
