About the Department
As a part of an interdisciplinary team, the successful candidate will provide Personal Health Program Services within the Community Clinical Services Division of the Kent County Health Department. Duties will include providing early intervention services (EIS), to Kent County residents living with HIV/AIDS by providing patient support and assistance, health literacy/education, navigation of the HIV care system and participating in community outreach. This is a case management type of position.
Work Schedule: Monday through Friday, 8am – 5pm. Will be required to work until 7pm on the 2nd and 4th Thursday of each month. Occasional weekend or evening work may be required. No Holidays.
Minimum Qualifications
High school graduate or equivalent. Previous work experience in a health care setting and knowledge of basic medical terminology and practices desired. Knowledge of HIV care and services desired. Computer literacy, including working knowledge of word-processing, spreadsheet, database management required. Exceptional oral and written communication skills and the ability to be highly efficient while ensuring attention to detail is required. Must interact positively and professionally with internal and external customers, and members of the public from widely divergent backgrounds. Ability to provide client service from an aware and client driven perspective and deliver excellent customer service to all clients while following HIPAA guidelines is essential. Must be able to provide services to clients in a variety of County locations and possess a valid, unrestricted Michigan driver’s license and have access to regular transportation in the performance of job duties.
Other Qualifications
All Kent County Health Department employees, regardless of position, are required to have their immunization status assessed and vaccinations brought up to date in accordance with the current CDC recommendations for health care professionals, with exceptions only as required by law. Mandatory vaccines include annual influenza vaccination.
