POSITION TITLE: CNA Coordinator - Vista


RESPONSIBLE TO: Administrative Director

SUMMARY: Responsible for creating a nurturing home-like environment for residents and providing care and services to meet their needs within an environment of hope, dignity and love. The CNA Coordinator works cooperatively and respectfully with residents, coworkers, families, medical staff, visitors and volunteers to create a positive environment. CNA Coordinator is responsible for guiding and coaching the CNA’s in creating a nurturing home for Elders and providing care and services to meet their needs. Responsibilities include but are not limited to oversight of the licensed assisted living facility and the care provided within, as well as compliance with all government regulations. Must be able to build and thrive within a team/family environment. 


ESSENTIAL JOB FUNCTION: Nursing Employee Assistance/Mentorship

Primary function is to assist in overseeing the function and performance of CAN/PCA working on the floor. CNA Coordinator, to “Mentor” new staff and orient them to the their roles both during their initial shifts as well as being a continued resource. 

  • Mentor/Supervision Given:
  • Supervises staff in assigned areas 
  • Work with staff to facilitate growth and build confidence. 
  •  Work a minimum of one shift with each new employee to facilitate staff orientation 
  • Oversee completion of CNA apprenticeship checklist 
  • Be available to staff to be called when assistance is needed while performing job duties
  •  Provide staff with guidance to contact appropriate administration, when necessary 
  • Provide staff with opportunities for coaching and correction
  •  Facilitate the hiring process for new staff and participates in interview process
  •  Assists in ensuring employee compliance with continuing education and certification renewal
  •  Assists in coordinating positions and scheduling nurses aides
  • Is expected to be part of the on call team


ESSENTIAL JOB FUNCTION: Personnel Coordination 

  • Function as liaison with SJU Human Resources to ensure employee files are up-to-date 
  • Represent The Vista at job fairs, Traditions lunches, and interviews 
  • Responsible for distribution of employee evaluations
  • Coordinate new hire paperwork
  • Follow-up on CPR certifications


ESSENTIAL JOB FUNCTION: Resident Care Coordination

To ensure that there is always an appropriate number of staff scheduled to care for residents while also ensuring that proper care is being provided by:

 • Ensure quality care by routinely rounding on residents. 

  •  Participate in community activities such as serving meals to residents to form relationships with staff and residents and ease burden of front-line staff. 

 • Audit resident charts periodically to ensure they are accurate and up-to-date.

 • Ensure compliance with all state regulations.

 • Participate in State survey preparations and inspections.

 • Participate in chart audits/ensuring documentation is being completed.

 • Assist with call calendar maintenance and scheduling. 

  •  Coordinate/assist with admissions in at The Vista

 • Attend and participate in care plan meetings with families

  • Attend Q & A Monthly




ESSENTIAL JOB FUNCTION: Administrative Assistance

To be a resource and provide skills in areas within the St. John's Corporation where training and experience may bring value. Inclusive but not limited to: 

  •  Provide accompaniment and transportation to Residents

 • Report to Administrative Director

 • Assist Administrative Director to complete various tasks as assigned

 • Gather initial inquiry information, set-up tour and conduct tour if needed.

 • Maintain inventory of equipment and medical supplies

 • Perform monthly bill audits to ensure accurate charges to residents for care provided

  • Be part of on call for sick calls from PCA/CAN
  • Assist Director with communication regarding upkeep and tidiness of Vista

ESSENTIAL JOB FUNCTION: Employee Responsibilities

Follows policy and procedures on annual influenza vaccinations 

  •  Completes all mandatory in-services

 • Performs other duties as assigned by the Administrator or Health Director of SkyRidge and WyndStone 



Requirements

REQUIREMENTS:

Minimum Requirements: Prior experience in human service or health care setting preferred. Montana Certified Nurse Aide certification preferred or must complete the SJU CNA Course and obtain certification within 120 days of class completion.      


Knowledge, skills, and abilities required:  

Knowledge and skills acquired through completion of the CNA training, complete Abuse Training and required training in food preparation, basic nutritional guidelines related to resident menu planning, basics of household financial management, housekeeping, infection control, and light maintenance. 


Skills/Abilities: Must possess good writing, verbal and observation skills. Excellent interpersonal communication skills. Exceptional organizational abilities. Basic computer skills and understanding of technology. 


Complexity/Difficulty: This position requires the ability to function independently in performing duties and be able to problem solve using good judgment with the Elder’s involvement whenever possible. The person needs to be able to respond to Elders needs in a timely and professional manner, recognizing the Elder’s rights. Incumbent performs job tasks following established procedures and works with Health Director/Administrator to identify and address problems or concerns within the team.


PHYSICAL DEMANDS:

Home-like environment with plants and animals present. 

  • Requires lifting, bending, walking and standing for prolong periods of time.  
  • Must be able to lift 25 pounds on a regular basis and up to 50 pounds occasionally.
  • Walking outside in inclement weather (going between Richardson Cottage/Main building/patio homes). 


WORKING CONDITIONS:

  • Work involves some exposure to unusual elements such as extreme temperatures, fumes, unpleasant odors, and/or loud noises. 
  • Work involves frequent exposure to hazards, blood borne pathogens, or physical risks, which require following basic safety precautions.
  •  Normal health care environment. 
  • Work involves exposure to plants and animals within the environment.
