Hospice RN – 3-Day Weekend Schedule

Position Summary:
We are seeking an experienced and compassionate Registered Nurse to serve as our Weekend On-Call Hospice RN. This role covers Friday 5:00 PM through Monday 8:00 AM, responding to patient needs after regular business hours. You will provide timely phone triage, make in-person visits as needed, and offer support to patients, families, and facility partners.

Key Responsibilities:

• Serve as the primary on-call nurse during the assigned weekend shift.
• Provide phone triage and support for patients, families, and caregivers.
• Make in-home or facility visits for urgent needs, symptom management, or admissions.
• Complete necessary documentation in accordance with agency and regulatory requirements.
• Communicate with the interdisciplinary team regarding patient status and care needs.
• Offer emotional support and education to patients and families.

Schedule:

• On-call from Friday 5:00 PM to Monday 8:00 AM
• Flexible scheduling of active visits within on-call window as needed.

To qualify for a Hospice RN (RN), with us:

• Education: Associate’s Degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred.
• Licensure: Current license to practice as a Registered Nurse (RN).
• Current CPR certification.
• Valid state-issued driver’s license, automobile insurance, and reliable transportation; willingness to travel.
• One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred.
