Behavioral Health Associates (BHA) is an educational and mental health organization. We offer an academic learning environment integrated with behavior support services for students diagnosed with emotional/behavioral disorders to empower the students in overcoming challenges and creating new stories in their lives.
We currently have an opening for a Social Service Coordinator to service our satellite location within Panther Valley School District.
JOB GOAL
The Social Service Coordinator assists the students, families, and school staff by establishing connections, curating resources, facilitating and providing support as needed.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provide mental health and support to individuals or families as needed.
- Work in collaboration with students and team to determine needs and goals.
- Assist teacher's with student behavior or emotional needs.
- Conduct exit meetings with sending schools to review student progress and transition planning for return.
- Follow-up with students and families who have transitioned to another level of care to promote continued success of the student, as needed.
- Maintain effective and cooperative relationships with teachers, students, administration, parents, peers, as well as the public.
- Provide referral information to necessary team members.
- Coordinate with students' mental health teams in order to assure continuity of care.
- Assist with the scheduling of necessary appointments for the student (i.e. psychiatric evaluations, outpatient therapy, etc.)
- Research and advocate for appropriate public assistance resources for students and families as needed.
- Assist student and families in utilizing resources curated by the case manager.
- Work with outside agencies and CASSP coordination to ensure that all of the students' needs are being met.
- Provide crisis intervention as needed during placement in the BHA setting.
- Ensure that all case file and other records strictly comply with policies, regulations, and procedures.
- Actively participate in ongoing training as needed in order to meet all certification standards and credentialing policies.
- Complete any other job related duties as directed by BHA's mental health administration.
REQUIRED SKILLS
- A strong desire to help others.
- Ability to listen and communicate effectively, both verbally and written.
- Strong interpersonal skills using tact, patience, and courtesy.
- Maintain a basic knowledge of community resources.
- Ability to problem solve.
- Possess time management and organizational skills.
- Knowledge of computer skills using Microsoft Word, Excel, email, etc.
- Experience with general office procedures and equipment, preferred.
- Ability to perform routine clerical duties as needed.
QUALIFICATIONS
- Bachelor's Degree in Human Services Field (Social Work, Counseling, or Psychology).
- Experience working with middle or high school aged youth, preferred.
- Possess a valid PA Driver's License.
- Ability to provide required clearances and negative TB test within the last four years.
