Job Summary:
This Position is Mobile within a radius 75 miles of Effingham Illinois.
Heal at Home is dedicated to delivering comprehensive wound care solutions to patients in the comfort of their homes. As a Veteran owned Small Business, we focus on service to rural and underserved communities. Our experienced staff utilize innovative techniques, including advanced amniotic membrane grafts, to optimize patient outcomes and satisfaction.
We are currently looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our mobile wound care team. The LPN will be responsible for assessing and treating patients with complex wound care needs in various settings, including private residences and healthcare facilities. This role offers the opportunity to work autonomously while contributing to a collaborative care environment, providing essential wound care services wherever they are needed.
Key Responsibilities:
- Perform thorough assessments, clean wounds, and apply appropriate dressings based on established protocols.
- Administer medications and treatments as prescribed, ensuring adherence to best practices and regulatory guidelines.
- Carefully document all patient interactions and clinical findings in electronic health records (Experience with Intellicure is a Plus!).
- Monitor patients for signs of infection and complications in wound healing.
- Provide education to patients and their families about wound care and prevention strategies.
- Collaborate effectively with other healthcare providers for comprehensive patient management.
- Maintain compliance with infection control and health safety standards at all times.
- Travel to assigned locations in a timely manner, demonstrating reliability and consistency.
- Manage inventory of wound care supplies and equipment efficiently.
Requirements
- Current and valid LPN license in Illinois.
- At least 1–2 years of clinical nursing experience, with a preference for wound care, home health, or long-term care settings.
- Wound care certification is preferred but company-paid training and certification will be provided.
- Must possess a valid driver's license and reliable transportation for travel to patient sites.
- Proficiency with electronic medical record systems is required.
- Strong communication, problem-solving, and organizational skills are essential.
- Ability to work independently and exhibit flexibility in a mobile healthcare role.
