The Location:

Kalele Care Services was founded in 2020 by Ohana Pacific Health to address the complex healthcare needs of the community with an emphasis on assisting Kupuna to live longer and healthier independent lives.

 

The Position:

Coordinates operations of care management services that bridge care across multiple settings and domains. Tracks and reports clinical and operational processes and outcomes. Directs and manages day-to-day Kālele Care Services (“Kālele”) operations, including directly supervising the Community Healthcare Workers (CHWs), Social Workers (LSWs), Nurse Care Managers (RN CMs), Office Coordinators, Schedulers, and any other program support personnel to ensure achievement of optimal outcomes through superior customer service, management of resources, and interdisciplinary collaboration. Trains and mentors staff, overseeing compliance with processes, documentation, billing, data inputs, and data reporting and analytics for key service line metrics. Works closely with Advanced Practice Providers (APPs) in ensuring optimal patient management throughout service duration. Provides social services support for patients on a consultative basis. Performs job duties in accordance with Ohana Pacific Health (OPH) core values and behaviors. Additional duties include:

  • Implement, manage, and evaluate current operational processes and procedures in accordance with the standards and procedures set out by the organization
  • Develops and monitors service line goals and objectives, including referrals, enrollments, client visits, process measures, key performance indicators, and documentation compliance. 
  • Develops and maintains databases and information on services and activities and produces monthly reports required for analysis of service line operations and outcomes

 

Requirements:

  • Bachelor’s degree plus any additional degree required for licensure in a specified clinical field
  • Two (2) years work experience in a clinical setting, preferably in a community setting/assignment with experience addressing social factors influencing health .
  • Prior leadership experience and/or experience to demonstrate ability to lead projects/ design programs. 
  • Experience with billing for clinical services and EMR integrations preferred
  • Staff management and coordination experience
  • Current Hawaii state licensure as an LSW (LCSW preferred), RN, OT, PT, SLP or related field.
  • Current medical/health screening including 2 step TB clearance
