Description
Position Summary:
This position requires a detailed-oriented nurse with MDS 3.0 experience who conducts federally mandated assessments of residents at a Long Term Health Care facility and has a desire to ensure quality in the care for long term care residents. Must be familiar with MDS 3.0, care planning and completing assessments in a skilled nursing environment.
Job Responsibilities:
- Gathers information on the facility’s current residents for present and future assessments including physical and mental status
- Assess the medical record and communicate to the healthcare team to create, revise and review health care plans for the current and incoming residents utilizing the nursing process, MDS, CAA’s and resident input
- Provides OBRA (Omnibus Reconciliation Act of 1987) assessment to ensure facility compliance with Medicaid or Medicare standards
- Required to act as the main communicator between the insurance professionals and management officials of the facility
- Planning, documentation, supervises the input of MDS assessments into the MDI, the submission of complied information and obtains receipt of validation
- Participate in daily review of new admission medical record, MD orders and nursing reports
- Ensure compliance with regulations utilizing the MDS schedule to plan and set MDS ARD
- Maintains and develops on-going MDS schedule
- Audit and analyze MDS data for accuracy
- Works in collaboration with the Unit Manager to schedule and facilitate careplan conferences
- Works in collaboration with the Unit Managers and unit staff for educational purposes and accuracy of documentation
- Conducts and/or participates in IDT meetings, care plan meetings and discharge meetings
- Educates members of the IDT as necessary in completion of the MDS and any changes in the MDS process
- Serves as a screening nurse as part of the admission screening team, reviews potential admissions with the team including consulting physicians if needed – documents necessary information
- Attends weekly Medicare meeting to ensure that eligible residents receive Medicare coverage as appropriate and ensures appropriate documentation is in medical record
- Works with residents, families as needed regarding questions on Medicare benefits and related issues (demand bill)
- Prepares information for Medicare appeals and/or insurance companies
- Observe and research as needed the pricing and effectiveness of services being offered and delivered to residents
- Works with the discharge planner in resolving outstanding resident issues
- Completes QA audits and Equip audits as assigned
- Recognizes link between MDS data and Quality Improvement measures.
- Maintains open communication with Medical Staff, Administrator, DON, ADON and Pharmacy consultant
- Completes PRI’s as assigned
- Performs function of a nurse when required
- Serves a Nursing Supervisor as needed
- Attends Nurse Management meetings, Patient Care meetings and any other meetings as assigned
- Performs other tasks as assigned
Qualifications
Education:
- Graduation from a regionally accredited or NYS registered four year college or university with a Bachelor’s Degree in Nursing, AND one (1) year of experience as a Registered Professional Nurse, which shall have been in a supervisory position; or
- Graduation from an approved three (3) year school of professional nursing AND two (2) years of experience as a Registered Professional Nurse, and (1) year of which shall have been in a supervisory position; or
- Graduation from a regionally accredited or NYS registered two (2) year college with an Associate Degree in Nursing AND three (3) years of experience as a Registered Professional Nurse, one (1) year of which shall have been in a supervisory position; or
- An equivalent combination of training and experience as defined within the limits of 1, 2, and 3 above.
Licensure:
Eligibility for a license issued by the State of New York to practice as a Registered Professional Nurse; Possession of the license at the date of hire.
AANAC Certification preferred
