SUMMARY OF OCCUPATION:
This position is considered supervisory/direct care in nature. The Program Supervisor is responsible for the implementation of the Individual Support Plans and the holistic care for each resident in their program. He/She is responsible for assuring adequate staffing to meet program needs in conjunction with the Program Director. Additionally this position is responsible for the supervision, scheduling, evaluation and discipline of Direct Care Counselors working at their facility.
Primary Duties and Responsibilities:
- Required to work various scheduled shifts; this is to include at least one (1) weekend shift per week. This position may require a supervisor to work flexible schedules in order to meet coverage needs or to complete job requirements. The Program Supervisor will also be on a one week on-call rotation (this includes an on-call paid stipend).
- Develops permanent staffing schedule, assuring that adequate staffing ratio is always maintained.
- Review and recommend approval/disapproval and forward requests for vacation and sick accruals to the Program Director for final approval. This does not include medical leave of absence.
- Submits staff vacancies to the Program Director for final review.
- Evaluates Direct Care Counselors after 6 month probationary period and annually on performance evaluation/review forms.
- Interviews Direct Care Counselor applicants in conjunction with the Program Director and recommends approval or disapproval of employment.
- Participates in all team meetings and attends all ISP meetings, day program and behavioral meetings.
- Assists with completing all consumer assessments and participates in the team process of establishing goals for their assigned residents.
- Assists Program Nurse to ensure all medical appointments are completed, current, and followed up when necessary according to DPW regulations.
- Completes all monthly and quarterly reports on timely basis for each resident in their program. (Due the 7th of the following month).
- Approves and submits all maintenance requests to the Program Director.
- Responsible for managing all house disbursements of petty cash and monthly reconciliation.
- Conduct a monthly staff team meeting.
- Attend regularly supervisor meetings (usually monthly).
- Rotate on-call schedule.
- Ensures that all programmatic documentation is current, complete, and orderly at program site.
- Initiates/conducts progressive disciplinary policy in conjunction with Program Director and Human Resources.
- Serves as liaison between families and interdisciplinary team on behalf of each consumer.
- Ensures implementation of all professional therapeutic plans.
- Ensures that all documentation is completed and disbursed to appropriate therapeutic professionals on a timely basis.
- Communicates daily activities schedule and adjustments to all pertinent staff.
- Ensures compliance with all 6400 Regulations relating to programmatic operations. (i.e. Individual Records, Medical Records, Fire Safety, etc.)
- Conducts formal program orientation and on-the job training program for Direct Care staff.
- Ensures all staff are meeting their targets and completing their mandatory 24 hour training annually.
- All other related duties as assigned by the Program Director.
Qualifications
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge of the practical aspects of training, and care of individuals with mental health and physical &intellectual developmental disabilities.
- Ability to implement written Individual Support Plans and Behavioral programs.
- Ability to write complete residential monthlies and quarterlies.
- Ability to effectively supervise and manage staff.
- Ability to manage the details of running a household.
- Ability to maintain accurate and complete records.
- Ability to effectively communicate with professional staff and medical personnel.
- Must be able to verbally and physically manage residents. i.e. lifting or restraining as necessary.
- Must have knowledge of county and state regulations including the Department of Public Welfare.
- Must be able to read, write and speak English in order to implement and read resident information as well as communicate with staff and residents.
- Maintain a current valid driver’s license with acceptable driver’s record from the DMV.
EXPERIENCE, EDUCATION AND TRAINING
- Bachelors Degree preferred, preferably in Human Services
- At least one (1) year experience working with handicapped individuals.
- Supervisory experience preferred.
