Job Summary:

Supervises, evaluates, and coordinates the activities of the team members.

What You Should Know About the Team Leader Role:

  • This is a full-time exempt position, M-F 8am-5pm, no weekends, no on call
  • This position manages the team for Ohio's Hospice Middletown OH,(Butler/Warren Counties) coverage areas and would include driving to facilities/team centers within that area as needed. 
  • We provide superior care and superior services to patients at their end of life journey.  Only those who have a heart for hospice will succeed.

Essential Duties:

Supervises, evaluates, and coordinates assigned members of the interdisciplinary team; assure continuity of care from admission to discharge or transfer to bereavement; serve as patient advocate and coordinator for other providers who are involved in the patient’s care.

Qualifications:

  • Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
  • Active Registered Nurse license in the State of Ohio without any board actions.
  • Minimum two years nursing experience and two years management experience preferred.
  • Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.) CPR Certified
  • Computer skills sufficient to properly document services and care
  • Travel may be required during daytime, nighttime or inclement weather.
  • Valid Driver's License with Safe Driving Record
  • State Minimum Automobile Insurance Coverage
