Job Summary

The Clinical Coordinator is assigned at the clinic level and is responsible for ensuring delivery of quality medical care to patients.   The Clinical Coordinator functions as a member of the multi-disciplinary health care team within the scope of his or her certification or licensure. In addition, the Clinical Coordinator is responsible for overseeing the daily activities of other clinical staff, such as scheduling, completion of assigned tasks and overall workflow of the clinical team.  The Clinical Coordinator, in conjunction with the Providers, RN Quality Coordinator, and Clinic Manager, assists in implementing quality initiatives, such as proper infection control, PCMH and other clinical quality directives.  The Clinical Coordinator role is designed to facilitate and enhance clinic workflow and is not intended to supersede any supervision required by the Nursing Practice Act for the practice of Professional or Vocational Nursing.

Duties and Responsibilities

  1. Provides clinical services and/or ensures that clinical services are covered by Clinical Staff.
    1. Collaborates with RN and medical director in order to maintain clinical expertise and competency (to include age specific competency) as well as demonstrates thorough knowledge of procedures to deliver quality primary health care.
    2. Documents delivery of health care and clinical processes in accordance with the Center’s policies and procedures in a timely, accurate, and consistent manner. 
    3. Educates the patient and family/significant others about diagnosis and the plan of care. Acts as a resource for facilitating communication when necessary.
    4. Coordinates care coordination program and teams.  Assists the care team with setting goals for quality assurance and best practices.  
    5. Coordinates the delivery of patient care provided by the health care team by overseeing clinical processes to ensure compliance.
    6. Interacts with health center personnel and assists providers to ensure optimal patient care and to obtain optimum patient flow and increased patient satisfaction.  
    7. Identifies and responds promptly to changes in patient’s condition and/or clinic environment.  
    8. Coordinates the timely processing of patient referrals, prescribed medications, and other identified needs of the patient.  
    9. Administers all medications accurately using the five (5) rights of medication administration, documenting medications according to health center policy and procedures.
  2. Collaborates and cooperates with Providers, RN and Practice Manager to provide clinical oversight for clinic and programs.
    1. Participates, advises, counsels, and coordinates disciplines and evaluations of all clinical staff within assigned clinic in conjunction with the Provider, Clinic Manager, RN and Regional Practice Manager.
    2. Assumes responsibility & accountability for ensuring clinical program compliance.
    3. May train and supervise other clinical staff, under the direction of the Providers, RN and/or Practice Manager.
    4. Participates in Compliance and Performance Improvement activities for continuous quality improvement and risk management purposes. 
    5. Serves as a resource and positive, proactive leader for the department.
    6. Participates in collaboration with the health care team in identifying, evaluating, and prioritizing patient and family needs to develop a plan of care.
    7. Participates in the evaluation of health care at the Center and in monitoring health outcomes of patients.
    8. Is responsible for appropriate infection control and employee health programs to include training staff and monitoring compliance.
    9. Completes medical record and clinical documentation audits and implements system changes for the clinical staff as necessary.
  3. Performs all other duties as assigned.

Knowledge, Skills and Abilities

Required

  • Knowledge of basic principles of health promotion, prevention and motivation
  • Knowledge of protocols in general and an ability to quickly gain specific knowledge of the Center’s current operational protocols
  • Knowledge of medical terminology and appropriate abbreviations
  • Demonstrates knowledge of the Texas Nursing Practice Act
  • Ability to read and comprehend medical instructions, short correspondence, and memos
  • Ability to document, with clarity, all information relevant to a patient’s needs
  • Ability to effectively present information in small group situations to patients, Center staff, and the general public. Ability to prepare statistical and administrative reports
  • Ability to work cooperatively with community agency personnel  to coordinate services for patients
  • Ability to use a glucometer and interpret the results
  • Ability to use office equipment including but not limited to computers, fax machines, telephones and copiers.   Ability to prioritize and allocate resources

Preferred

  • Spanish speaking and writing skills

Credentials and Experience

Required

  • Registered Nurse (RN)  
    • Must have current, unrestricted license from the Texas Board of Vocational Nurse Examiners or Appropriate State Board if not in Texas
  • Cardiopulmonary Resuscitation (CPR) for Healthcare Professionals 
  • One (1) year of management experience

 - Will consider Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) - Depending on experience
