- Master's degree in a behavioral health related field.
- Bachelor's degree in behavioral health related field and (2) years of behavioral work experience.
- High School Diplomas or equivalent and 4 years of behavioral health work experience.
- Consumer Support: Engage with consumers in the community, providing supportive counseling, skill development, and other necessary services in line with established policies, procedures, and clinical protocols.
- Team Collaboration: Work closely with the consumer’s Prescribing Clinician, Nurse, and other clinical team members to ensure the timely development and implementation of individualized service plans and crisis plans, including all necessary assessments.
- Coordination of Care: Communicate and coordinate with all parties involved in the consumer’s treatment, including inter and intra-agency staff, through meetings, calls, letters, and service delivery, as indicated in the consumer’s service plan.
- Access to Services: Assist consumers and their families in accessing additional needed services and obtaining entitlements, including identifying funding sources for services.
- Transportation: Provide transportation to consumers for necessary services, as outlined in their service plan.
- Primary Contact Responsibility: Serve as the primary contact for a designated percentage of team consumers, ensuring accurate, thorough, and current documentation in the medical record, maintaining updated client schedules, and acting as the first point of contact for consumers and their families.
- Documentation: Accurately document all consumer events in accordance with policies, procedures, and clinical protocols.
- Team Participation: Actively participate in daily team meetings and any additional treatment team or communication meetings as required.
- Fidelity Compliance: Collaborate with team members to ensure compliance with ACT (Assertive Community Treatment) fidelity measures.
