This position has regular contact with or control over Indian Children and Vulnerable Adults. In compliance with Federal Law, this position will not be filled by an applicant with any felonious offense or any of two or more misdemeanor offenses under Federal, State, or Tribal law involving crimes of violence; sexual assault, molestation, exploitation, contact, or prostitution; crimes against persons; or offenses committed against children.


The Community Health Representative (CHR) is responsible for providing health related services to aid in maintaining an effective and responsive Community Health program. This position involves assisting all eligible patients to gain access to health related services while always maintaining the highest level of confidentiality.

DUTIES AND RESPONSIBILITES:

  • Transport patients to medical appointments and services.
  • Assist patients in locating providers for medical and dental services.
  • Assist patients with applying for available health benefits and processing health bills.
  • Provide referrals to other agencies or tribal programs for health related services and encourage involvement in all available services.
  • Provide advocacy on behalf of patients during interviews with other agencies.
  • Provide follow-up on special screening clinics conducted at the Tribal Clinic.
  • Provide basic health education to patients and the community.
  • Provide 72 hour notifications to CHS as required for individuals obtaining emergency care.
  • Assist providers in preparing patients for exams, charting, and vitals.
  • Attend inter-tribal and other government health conferences and trainings.
  • Provide health related assistance and referrals to elders.
  • Provide timely communication and report necessary issues immediately to the health care team working with the patient.
  • Remain current with all other programs, services, and resources available for clients and families.
  • Maintain confidential records and files and provide timely documentation as required.
  • Maintain the highest level of confidentiality at all times.
  • Perform other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES:

  • Knowledge of HIPAA laws and confidentiality requirements.
  • Knowledge of the Nisqually Tribe and community.
  • Knowledge of application procedures used by various medical services established for their general public.
  • Knowledge of medical supportive services available to respond to community needs.
  • Knowledge of and the ability to relate with the unique health needs of a Native American community.
  • Ability to maintain the highest level of confidentiality at all times.
  • Ability to safely lift, pull, and push up to 50 pounds and safely and effectively transfer patients as required.
  • Ability to communicate effectively with community members, visitors and staff.
  • Ability to travel and be on call in order to carry out the responsibilities of the position.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to manage and compile data and other records accurately.
  • Ability to interact with Nisqually families and community members and to gain their confidence and trust.
  • Ability to demonstrate a passion for improving the welfare of others.

QUALIFICATIONS

  • High School diploma or GED equivalent.
  • Must successfully complete CHR training course within 1 year of hire.
  • Must be compliant with HIPAA rules and regulations as it relates to job duties and responsibilities.
  • Must possess a valid driver's license and be insurable under the Tribe's insurance policy.
