We need individuals who are passionate about making a difference by helping others. 

 

We are seeking a Care Coordinator – Clinics to support clinical area by actively assisting with coordination of care to ensure continuity while receiving services. Collaborate with clinical teams, linking consumers to community-based resources and supports. Work closely with clinical treatment teams, identifying barriers to care and assist care team with implementation of supportive treatment plans. 

 

This position is 40 hours a week, Monday through Friday, 8:00 am to 4:30 pm. There are no weekends, on call, or holiday requirements for this position. This assignment is in person at 790 Fuller AVE NE Grand Rapids, Michigan 49503. 

 

MINIMUM QUALIFICATIONS: 

• Bachelor’s degree in human services related field (for example, Social Work, Education, Psychology, Sociology).

• At least one year of satisfactory work experience working with individuals with intellectual and/or developmental disabilities, mental illness or substance use disorders. 

• Thorough working knowledge of professional ethics and standards of practice.

• Computer skills including word processing, spreadsheets, email, database management, and data entry skills.

• Demonstrated experience communicating clearly and concisely in stressful situations.

• Excellent communication skills, both verbal and written.

• Excellent customer service skills.

• Ability to work effectively in a multicultural environment and skills and capabilities of bridging language/cultural barriers to Network180 services within diverse communities.

 

PREFERRED QUALIFICATIONS: 

• Three years post undergraduate experience working with individuals with mental illness and co-occurring disorders.

• Spanish/English bilingual.

• Lived experiences with mental illness/developmental disabilities/substance use disorders valued.

 

Any combination of education, training and experience providing the required knowledge, skills and abilities to perform the essential functions of the job will be considered. Equivalency for relevant education is two years of related work experience equals one year of education.
