Description

Provides direct care to terminally ill patients and their families while functioning within a medically approved plan of care. The Certified Nurse Assistant will deliver services under the direction of the interdisciplinary team and be supervised by a registered nurse.

Requirements

Specific to Job Functions:


1.Fosters and maintains an effective working relationship between co-workers, patients, and other professional members of the community

2. Follows an individualized Plan of Care established by the interdisciplinary for each patient

3. Successfully demonstrates the ability to provide the following care(s) to patients:

  •    Bathing (shower, partial, full bed bath)
  •    Oral Care (for an alert patient, an unresponsive patient, and a patient with dentures)
  •    Shaving (with disposable and electric razor)
  •    Vital Signs (blood pressure, pulse, temperature, and respirations)
  •    Basic Nail Care
  •    Shampoo Patient’s Hair (in shower and in bed)
  •    Use Appropriate Dressing Techniques (for an ambulatory, wheelchair bound, bedbound and unresponsive patients)
  •    Provide Effective Skin Care
  •    Knowledge of Proper Toileting Techniques
  •    Knowledge with Proper Colostomy Care
  •    Knowledge with Urinary Bag Devices
  •    Appropriate Positioning and Body Alignment
  •    Range of Motion Activities
  •    Safe Transfer Techniques
  •    Reporting Change in Patient Condition to RN Case Manager
  •    Notify RN Case Manager when Patient Complains of Pain
  •    Basic Homemaker Services
  •    Light Housekeeping Services
  •    Laundering of Personal Clothing and Linens
  •    Meal Preparation/Feeding
  •    Companionship

4. Gathers data as assigned by the RN Case Manager

5. Assists with orientation of new CNA staff 

6. Provides support and peer to peer education for trainees

7. Immediately reports any changes to the RN Case Manager or designated charge person

8. Reports unusual conditions, observations, and pertinent information to the RN Case Manager

9. Understands/provides basic principles of palliative care/pain management within the Scope of Practice

10. Provides personal care as assigned by the RN, which may consist of, but is not limited to, Certified Nurse Assistant duties,      companionship, light housekeeping, etc.

11. Complies with all documentation policies/procedures, which include documentation at bedside

12. Completes appropriate documentation in a legible and timely manner

13. Demonstrates good judgment with proper decision-making abilities

14. Completes monthly and annual trainings as assigned by the Governing Board, Human Resources and/or Director of Nursing

15. Other duties as assigned

Additional Functions:

  1. Adheres to patient rights, abuse reporting and confidentiality policies
  2. Attends scheduled staff meetings and/or in-service trainings
  3. Adheres to departmental dress code requirements related to uniform, personal grooming, and other attire
  4. Participates in Performance Improvement activities as requested
  5. Demonstrates reliability by making every attempt to report to work on time, providing proper notification for absence or tardiness and following company policies related to time off requests
  6. Assumes responsibility for on-going personal development and continuing education
  7. Seeks guidance from a higher authority as needed

Physical/Safety Requirements:

  1. Must have the strength and endurance required for standing, walking, lifting, bending, and pushing on a consistent basis up to 50 lbs.
  2. Must utilize appropriate durable medical equipment as needed in a safe manner
  3. Must use appropriate body mechanics for CNA duties as identified in the scope of practice
  4. Category 1 for potential exposure to blood/body fluids. (Requires the performance of procedures or other tasks in the work routine that involve an inherent potential for mucous membrane or skin contact with blood, body fluids or tissues, or a potential for spills or splashes of them. Protective measures should be used while engaged in Category 1 tasks with exposure potential.)
  5. Utilizes appropriate PPE as needed
  6. Follows the designated plan of action in the event of a fire or other emergency
  7. While performing the duties of this job, the employee is regularly exposed to fumes or airborne particles, work-related stresses, and driving hazards
  8. The individual may be required to drive to meetings with clients or health care professionals or patient’s homes 
  9. The noise level in the work environment is usually minimal

Qualifications and Experience:

  1. Must be currently certified as a Certified Nurse Aide by the State Board of Nursing in the State of Nevada
  2. Prefer 1 year experience as a CNA in hospice or home health or geriatric care
  3. Must be able to demonstrate basic CNA qualifications and successfully pass initial direct and indirect supervision competency items prior to direct care with a patient
  4. CPR/First Aid Certified
  5. The Home Health Aide may also be referred to as a CNA and will have the ability to perform basic homemaker services as well
  6. Successful completion of a background check pursuant to NRS 449.188 and NRS 179A.100 along with a pre-employment drug screen and physical, TB testing, previous employment and reference checking, and an MVR (may include verification of education)
  7. Read, speak, write, and understand the English language
  8. Must be able to handle frequent interruptions
  9. Must be able to cope with emotionally charged situations encompassing, patients, families, and other caregivers
  10. Must have a valid driver’s license, reliable transportation, and current auto insurance
