Job Summary:
The Personal Services Coordinator is responsible for coordinating and maintaining scheduling for home health and hospice services to ensure timely and appropriate care delivery. This position supports clinical operations by managing staff schedules, matching clinician availability with patient needs, and facilitating effective communication among team members, patients, and families. The Personal Services Coordinator supervises Personal Care Attendants who provide direct in-home care to hospice and long term care patients.
Essential Job Functions/Responsibilities:
- Coordinates and maintains schedules for home health and hospice clinicians and paraprofessionals, ensuring patient visits are scheduled according to care plans, regulatory requirements, and patient preferences.
- Monitors staffing levels and coverage for planned and unexpected absences, making timely adjustments to ensure continuity of care.
- Communicates effectively with patients, families, and caregivers to confirm appointment times and changes.
- Collaborates with clinical leadership and intake/admissions staff to prioritize urgent or high-acuity cases.
- Communicates schedule changes, patient concerns, and urgent needs promptly and effectively to field staff and clinical managers.
- Maintains patient and staff confidentiality in compliance with HIPAA and organizational policies.
- Documents scheduling activities accurately in the EMR and scheduling systems.
- Provides direct supervision, guidance and mentorship to Personal Care Attendants, conducts annual performance evaluations and supports professional development.
- Ensures Personal Care Attendants adherence to care plans, required training and certifications in accordance with organization policies and quality standards.
- Assists with onboarding and orientation of new Personal Care Attendants and ensures continued competency.
- Ensures continued competency of Personal Care Attendants through home supervisory visits and in-services.
- Maintains accurate records of Personal Care Attendant schedules, training, incidents and supervisory visits.
- Report incidents, grievances, and care concerns according to policy and regulatory requirements.

Position Qualifications:
- High school diploma or equivalent required; associate’s degree or healthcare-related coursework preferred.
- Minimum of one (1) year of scheduling experience in a health care setting, preferably in home health or hospice.
- Proficiency in scheduling software and EMR systems.
- Strong organizational and problem-solving skills.
- Excellent interpersonal, communication, and customer service skills.
- Ability to prioritize and manage multiple tasks in a fast-paced environment.
- Attention to detail and commitment to accuracy.
- Must maintain a high level of confidentiality.
- Requires prolonged sitting, some bending, stooping and stretching.
- Requires eye-hand coordination and manual dexterity sufficient to operate a keyboard, photo-copier, telephone, calculator and other office equipment.
- Requires normal range of hearing and eyesight to record, prepare, and communicate appropriately.
- May occasionally be in a position to lift and/or carry an item weighing up to 25 pounds.
