- Intervenes and evaluates clients, establishes diagnosis, as needed. Assists int he development of the individual case plan collaboratively with clients, their guardian(s) (if applicable), their child welfare case worker (if applicable) and the QMHP by defining goals, specific outcomes, and time frames for treatment. Monitor’s client’s behavior.
- Completes IM+CANS and related clinical assessment tools for consumers referred for the FAC program, and others as assigned, in need of a mental health evaluation for child welfare requirements.
- Provides crisis intervention to stabilize client’s condition by utilizing short-term crisis intervention techniques and/or referral to other support services.
- Carries a caseload as assigned by direct supervisor.
- Intervenes to change attitudes, beliefs and behaviors of consumers to improve individual and family level of functioning.
- Maintains networking and referral contacts with other human service providers both internally and externally while assessing barriers for successful engagement, treatment, and discharge.
- Completes required monthly direct service hours/billing requirements.
- Demonstrates active involvement in team/program meetings, supervision, and agency/community events and activities as assigned.
- Facilitates comprehensive consumer care both in office and in a consumer’s natural environment (home, school, community, etc.) by providing case management services including but not limited to: advocacy services (vocational training, supported employment, public entitlements, and benefits acquisition) and linkage with community resources (dental, vision, clothing, hygienic products, and services as determined necessary by the assessment); and community support services including but not limited to: parenting education, individual and family counseling, and advocacy with Child Welfare and Legal systems throughout the clients’ stages of treatment with a focus on engagement, care coordination, and discharge planning.
- Provides and arranges transportation services to consumers by connecting with internal and external resources such as a primary care provider.
- Facilitates Parenting Education and Caregiver Support groups and individual services in home, in office, and virtually.
- Ability to screen, assess and provide interventions for consumers that are exhibiting symptoms of trauma.
- Participates in DCFS related training as scheduled and assigned including but not limited to Financial Literacy and Be Strong Families trainings.
