About Us
Legacy Hospice is seeking a compassionate and experienced Visit Registered Nurse (VRN) to join our dedicated team. In this role, you�ll provide vital care and support to patients in their homes, supplementing our RN Case Managers and ensuring timely responses to urgent needs.
What You�ll Do
- Perform focused nursing assessments and address acute needs of hospice patients as directed
- Ensure continuity of care through effective communication with the interdisciplinary team
- Complete initial and comprehensive assessments of patients and families to determine hospice needs
- Develop and revise individualized care plans in collaboration with the patient and caregiver
- Administer prescribed medications and treatments, and educate families on care techniques
- Counsel patients and families on symptom management and end-of-life issues
- Maintain accurate and timely clinical documentation
- Assist patients with activities of daily living and promote comfort-focused care
- Respond promptly to changes in patient condition or caregiver concerns
- Collaborate with case managers to ensure needed supplies and services are provided
- Participate in interdisciplinary team meetings and quality improvement initiatives
Qualifications
- Registered Nurse (RN) licensed in the state of employment
- Certified Hospice and Palliative Nurse (CHPN) and/or BSN preferred
- Minimum 1 year of hospice nursing experience required (2+ years preferred)
- Active Basic Life Support (BLS) certification
- Strong written and verbal communication skills
- Self-motivated and capable of working with minimal supervision
- Valid driver's license, reliable vehicle, and proof of insurance
