Are you passionate about helping people transition from institutional care back into the community? Join our mission-driven team as a Community Care Manager and support individuals on their journey toward independence and wellness.

What You'll Do:
As a Community Care Manager, you’ll work one-on-one with clients transitioning from nursing homes and long-term care facilities. You’ll create and manage personalized care plans, coordinate services, attend appointments, advocate for client needs, and provide hands-on support, from housing searches to setting up SNAP benefits. Every day is different, meaningful, and impactful.

Requirements

QUALIFICATIONS

 What You’ll Need:  

?? Master’s in Social Work, Counseling, Psychology, or a related field  

?? 3+ years of experience supporting individuals with mental health, substance use, or complex medical needs  

?? Valid driver’s license & reliable vehicle  

?? Strong communication, advocacy, and organizational skills  

?? Passion for empowering others 

Compensation & Benefits:

• Base Salary: $60,000 per year 
• Paid Time Off: 11 Vacation days, 12 Holidays days, 3 sick days, 3 personal      days
• Illinois Paid Leave: 40 hours (accrued)
