Lead Transition Teacher Full Time Pay Rate: Commensurate with Experience/210 Days EdAdvance on Post University Campus
Job Purpose : The Lead Transition Teacher oversees the daily instructional operations of a Post University based transition program for students who are between the ages of 17-22. The position entails providing services to students of all learning exceptionalities to make progress on their individual academic, social/emotional, and transition IEP goals and objectives.
Qualifications :
Master’s degree in teaching from an accredited institution in Special Education.
Current Connecticut teaching license in required content area (265 or 165 or 065)
Must have proven the ability to report to work on a regular and punctual basis
Valid Connecticut driver’s license
Perform all other related work delegated or required tasks assigned
Major Responsibilities :
Complete all case management tasks and responsibilities for students who are assigned to your caseload
Develop and implement all components of the Individualized Educational Program (IEP) for students on your caseload and others as appropriate
Provide research-based specialized instruction to address the instructional goals and objectives contained within each student’s IEP
Coordinate the delivery of special education services in each student's IEP across all settings
Serve as the primary contact for parents/guardians/Post University/District/and other deemed part of the students team
Provide direction to community coaches as directed
Provide consultation to Post University staff regarding classroom adjustments, instructional modifications, adaptive equipment, behavior modification plans and other similar instructional interventions to meet the needs of students with disabilities.

The Lead Transition Teacher works under the supervision of the UPP Coordinator & Director of Regional and Transition Services, and/or designee.
