About the Role:
The Chaplain at the Sulphur Springs Hospice Branch plays a vital role in providing spiritual and emotional support to patients, their families, and the healthcare team within a hospice care setting. This position is dedicated to addressing the diverse spiritual needs of individuals facing end-of-life challenges, ensuring compassionate care that respects patient rights and dignity. The Chaplain collaborates closely with interdisciplinary teams to develop and implement holistic care plans that integrate pastoral counseling and grief support. By fostering an environment of stewardship and volunteer engagement, the Chaplain enhances the overall patient and family experience. Ultimately, this role contributes significantly to the mission of delivering comprehensive, patient-centered hospice care in the community.
As a Hopestone Hospice Chaplain, you will:
Assess hospice patients and support systems to identify spiritual needs as evidenced by POC, documentation, clinical records, and IDG conference minutes.
Provide support for the spiritual needs of the patient/family.
Attend interdisciplinary team meetings and participate in the patient care planning process.
Use effective interpersonal relations and communication skills; facilitates the use of these skills by other team members to achieve desirable outcomes.
Meet mandatory continuing education requirements of the agency.
Demonstrate commitment, professional growth and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
Promote Agency philosophy and administrative policies to ensure quality of care.
Demonstrate understanding of coordination and continuity of care by collaborating with the IDG.
Accept clinical assignments that are consistent with education and competence to meet the needs of the patients/families.
Requirements
One year of experience as a Chaplain in health care preferred
One or more years of Hospice experience required
CPR Certification
Valid Driver’s License
Valid Auto Insurance
