As an Advanced Nurse Practitioner at Transitions Care you will be responsible for the direction and oversight of the care and treatment to hospice patients rendered by the hospice care team and provide oversight for Plan of Care and all changes to the Plan of Care. Implement the Plan of Care provisions to meet the patient’s medical needs. Travel, as necessary.


Major Responsibilities

 

  • Consult and work with the patient's attending physician. 
  • Assure compliance with local, state, and federal laws, Medicare regulations, and HIPAA.
  • Accountable for direction and oversight of the care and treatment of palliative and hospice patients.
  • Implement the plan of care to meet the patient’s medical needs, assess and reevaluate the patient’s medical needs on a regular basis, and manage related expenditures in a fiscally responsible manner.
  • Assumes responsibility for meeting regulatory and other mandatory requirements.
  • Reviews and interprets diagnostic results in a timely manner.
  • Prepares and maintains clinical documentation utilizing company EHR.
  • Routine visits focusing on primary care and prevention.
  • Complete and document all Hospice Face to Face encounters 
  • Maintain clinical records, statistics, reports, and records for purposes of evaluation and reporting of agency activities as prescribed and in compliance with local, state, and federal laws.
  • Function as the primary on-call clinician, functioning as the lead responder for all clinical escalation needs.
  • Address urgent symptom concerns via phone or in person (if required)
  • Complete face to face evaluations with documentation within 24 hours of completed visit
  • Prescribe and adjust medications as appropriate and within scope of practice and licensure
  • Communicate with on-call RNs and support staff to ensure coordinated care
  • Confer with medical directors or physicians as needed
  • Complete medical record reviews for quality assurance functions as appropriate

Requirements

Education and Experience

  • Master’s Degree in Nursing
  • Current license to practice in the state of employment and Controlled Substance if required by the state of employment.
  • Active DEA License with no restrictions
  • One (1) + year of home care or hospice experience
  • Knowledge of the principles and practice of hospice and palliative care, working knowledge of geriatric medicine.
  • Knowledge of hospice and palliative care with emphasis on control of symptoms associated with a terminal illness.
  • Hospice and Palliative Care certification preferred.
  • Knowledge of the local medical community and the ability to enhance the hospice's visibility and reputation with practicing physicians and hospitals.
  • Ability to collaborate with the patient's primary physician to interpret the hospice program and affect optimum medical care.
  • Ability to work within an interdisciplinary setting.
  • Understanding of Hospice philosophy, principles of death/dying.
  • Compliance with accepted professional standards and practices.
