About the Role:
The Home Health PRN Weekend Registered Nurse (RN) plays a critical role in delivering high-quality nursing care to patients in their homes during weekend shifts. This position ensures continuity of care by performing comprehensive assessments, developing and implementing individualized care plans, and monitoring patient progress in a home health setting. The RN collaborates closely with interdisciplinary teams, patients, and their families to promote optimal health outcomes and patient safety. This role requires adaptability and strong clinical judgment to manage diverse patient needs independently while adhering to regulatory standards and organizational policies. Ultimately, the RN contributes to enhancing patient quality of life by providing compassionate, skilled nursing care in a community-based environment.
As a Home Health RN, you will:
Initial and ongoing comprehensive assessments and treatment on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs.
Apply specific criteria for admission and re-certification to home health care to establish appropriate levels of care and the patient’s eligibility.
Implement, develop, and document the plan of care to ensure quality and continuity of care and recommend revisions to the plan as necessary.
Consult with and educate the patient and family regarding self-care techniques, nutrition, and dietary needs.
Determine scope and frequency of services needed based on acuity and patient/family needs. Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
Initiate appropriate preventative and rehabilitative nursing procedures.
Applies concepts of infection control and standard precautions in coordinating and performing patient care activities to prevent contamination and transmission of disease.
Provide clinical directions to the Home Health Aide and LPN/LVN to ensure quality and continuity of service provided.
Prepare clinical and progress notes that demonstrate progress toward established goals.
Inform physician and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care.
Monitor assigned cases to ensure compliance with requirements of third party payor.
Provide effective communication to patients, family, team members, and other health care professionals.
Requirements
Current RN License
Valid Driver’s License
CPR Certification
Valid Auto Insurance
At least 2 years of Home Health RN experience preferred
HomeCare HomeBase experience preferred
