We are seeking a dynamic and experienced Resident Care Coordinator to join our dedicated leadership team, where you’ll play a pivotal role in providing exceptional, compassionate care to our residents, mentoring our talented team, and fostering positive, enriching experiences every day.


  As a Resident Care Coordinator, you will:

· Partners with the Program Manager and DAL to develop, implement, and oversee individualized resident care plans, ensuring they reflect the unique needs and preferences of each resident and align with all regulatory requirements.

· Be a steward of best practices by regularly reviewing and updating our policies and procedures, offering valuable insights to the Program Manager and DAL to enhance our standards of care.

· Empower our direct care team through effective coaching, mentorship, and training, providing constructive feedback to the Program Manager to support their professional growth.

· Foster a culture of ownership and accountability by delegating responsibilities clearly and effectively.

· Serve as a vital link between residents, families, and our care team, proactively communicating changes in resident needs and facilitating smooth transitions when a higher level of care is required. You will also lead crucial conversations through resident and family meetings to ensure ongoing satisfaction and address any concerns.

· Ensure optimal staffing levels across all shifts by managing schedules efficiently (including the use of agency staff when necessary), providing crucial support to direct care staff in managing their daily tasks, and stepping in to provide direct care when needed.

· Be an advocate for our care team by ensuring they have the necessary resources and tools to excel in their roles.

· Lead the onboarding experience for new team members and champion ongoing training initiatives that cover company policies, job responsibilities, and regulatory compliance.

· Support the professional journey of our staff by recommending relevant training opportunities and ensuring all state-mandated requirements are met and documented.

· Contribute to a welcoming and well-maintained environment that promotes the well-being of our residents.

· Maintain open and collaborative communication with the Program Manager and DAL, providing valuable insights and contributing to the achievement of shared goals.

· Embrace opportunities to lead and participate in special projects that enhance our services and community.



  


Requirements

  

· Current licensure as a Licensed Practical Nurse (LPN).

· A minimum of two years of dedicated experience in geriatric care.

· Exceptional communication skills, both written and verbal

· Proven ability to organize and manage multiple priorities effectively.

· Demonstrated sound judgment and problem-solving skills.

· Current CPR certification or a commitment to obtaining it within one month of hire.

· A valid Maine driver’s license.
