JOB SUMMARY
Under the direct supervision of a Clinical Market Leader, the Clinical Resource position will support local clinical leaders at agencies located throughout Texas and the Midwest.In association with those local clinical leaders, the Clinical Resource will be responsible for supporting the local agencies through Field nurse support and education. May also be utilized for education and operational assistance in temporary DCS role and back-office support.

DUTIES & RESPONSIBILITIES
Responsible for providing a high level of support and guidance in the establishment of programs, policies and practices that ensure that all Cornerstone Home Health and Hospice agencies provide exceptional clinical care in compliance with State and Federal Regulatory requirements.
Educates and assists Cornerstone’s local clinical leaders to assure field clinicians are working at the highest level. Works closely with Cornerstone’s compliance resources to ensure appropriate and effective survey preparedness.
Assists local leadership in developing and implementing clinical processes to monitor and measure all activities related to clinical, education, and performance improvement.
Provides support regarding the direction and coordination of educational programs, orientation, new hire competency systems, annual skills competency and on-going in-service educational program development.
Educates and trains nursing supervisors to ensure documentation is complete and consistent with legal requirements.
Helps develop initiatives and monitor agency process and progress in areas including, patient safety, risk management, infection control programs.
Educates and trains nursing supervisors to ensure documentation is complete and consistent with legal requirements. Additionally, a clinical resource may be asked to step into patient homes for observations, evaluations, and mock surveys.
Resource will be expected to travel extensively (4 weeks per month)

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

Minimum Experience

A minimum of five years’ Home Health and Hospice experience
Understanding of the legal and regulatory framework governing the home health industry
Ability to work with all levels of management on all aspects of therapy operations
Will be required to show proof of a valid driver license, motor vehicle insurance, and a 2-step TB screen (giving within the past 6 months of date of hire).
Understanding of the legal and regulatory framework governing the home health/hospice industry
Ability to work with all levels of management on all aspects of clinical operations
Verbal and written communication skills necessary to explain complex and/or confidential information
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required.
Communicates with community health related persons to coordinate the care plan.
Additional Duties
Participating in the HHA's quality assessment and performance improvement program and HHA-sponsored in-service training.
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services are available.
Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
Supervises Licensed Practical Nurses (LPNs)/ Licensed Vocational Nurses (LVNs) and/or Home Health Aids (HHAs)/Certified Nursing Assistants (CNAs), as directed.

JOB REQUIREMENTS
Must have graduated from an approved school of nursing and be licensed in the state where practicing.
Minimum of two years of experience, at least one of which is in the area of public health or home care nursing is preferred.
Maintains a current CPR certification.
Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Self-directed and able to work with minimal supervision.
Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
Shows ability to organize and prioritize workload independently.
Management experience not required. Responsible for supervising home health aides.
Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
