Job Description:

Assist pharmacists in conducting post-discharge medication reconciliation to provide continuity of care, identify medication related concerns, and reduce readmissions.

Key Responsibilities and Tasks:

  • Perform post-discharge medication reconciliation using inpatient discharge summaries, PCP medication lists, and prescription fill history.
  • Identify medication related discrepancies and optimizations.
  • Document all clinical findings and recommendations in electronic medical record.
  • Escalate clinical concerns to supervising pharmacists.
  • Send completed documentation to PCP or specialists through electronic medical records or fax.

Skills and Experience Required:

  • Certification as a Pharmacy Intern
  • Attention to detail, excellent time management and organizational skills, and ability to follow precise instructions
  • Excellent written communication skills
  • Self-motivated
  • Basic electronic medical record knowledge and ability to quickly learn new software
  • Proficient in Microsoft Excel

Salary: $25-$30 hourly
