This role is all about setting the tone for excellent care. You'll be the first face our patients see, the calm in the chaos, the educator, the advocate, and the foundation for their care journey.

Responsibilities

What you'll be doing !

• Conduct Start of Care (SOC) visits
• Complete OASIS documentation accurately and timely
• Perform skilled nursing assessments
• Educate patients and caregivers
• Collaborate with interdisciplinary teams
• Ensure compliance with care plans and regulations

Requirements

What we're looking for

• Active Registered Nurse (RN) license in the state of Michigan
• OASIS Experience a plus
• Minimum of one year of nursing experience, preferably in home health care or acute care setting.
• Strong assessment and documentation skills.
• Reliable transportation
• Positive energy, compassion, and professionalism
• Excellent communication and organization
