JOB SUMMARY:
The Housing Stabilization Pilot (HSP) Coordinator will carry out the mission and values of CCAP by working alongside DHS as a liaison to support families in need of stable housing. This role is responsible for determining eligibility for families at risk of eviction or requiring move-in assistance, processing all HSP applications, and ensuring all supporting documentation is complete and accurate. The coordinator will maintain detailed records of every application, uphold compliance standards, and provide clear communication between CCAP, DHS, and partner agencies. The ideal candidate will be highly organized, detail-oriented, and committed to helping families achieve housing stability through compassionate and efficient service.
WORK SCHEDULE DEMANDS:
- This is a full-time position, 40 hours a week.
- Due to the nature of the job, this individual must be flexible with their schedule to accommodate the client population and community events.
Requirements
REQUIRED QUALIFICATIONS:
- Bachelor’s degree in social work or business finance and/or 3-5 years’ experience in housing case management programs, preferred.
- Experience working with Homeless Management Information System (HMIS), preferred.
- Excellent communication, organizational and decision-making skills.
- Ability to work independently and as part of a team.
- Proficiency in documentation and database systems, specifically Microsoft programs.
- Must possess excellent written and oral abilities.
- Able to handle multiple tasks and deadlines.
- Bilingual in Spanish, preferred.
- Provide/maintain all required immunizations and/or vaccinations.
- Complete/maintain all required background checks.
KEY RESPONSIBILITIES:
Programmatic:
· Review Housing Stability Pilot (HSP) applications and determine eligibility for funds.
· Approve/deny and process up to 20 application packets a week from Department of Human Services, with first review occurring within 48 hours of receiving.
· Enter DHS Housing Stability Pilot approve funding applications into HMIS (Homeless Management Information System) when available.
· Ability to speak effectively before groups and consumers, representatives of other agencies and CCAP employees.
· Collaborate with community partners and internal teams to coordinate services.
· Support case managers working with clients to ensure all application requirements are met.
· Manage multiple cases, deadlines, and compliance requirements.
· Attend monthly HSP office hours to help answer questions.
· Participating in Professional Housing Committee meetings
· Attend bi-weekly meetings with DHS staff on HSP program and attend all training meetings needed to fulfill program
· Generate statistical reports as needed for program.
· Maintain open lines of communication with DHS Housing Stability Pilot workers and management team.
· Perform other duties as assigned to support program success.
Financial:
· Complete approved payment processes to landlords and/or business entities in a timely manner.
· Be the liaison for approved cases with landlords, utility company and other organizations receiving payments to ensure the payment is received.
· Track spending on housing funds daily.
