POSITION SUMMARY:
This position provides a combination of physical therapy services and case management to eligible infants and toddlers and their families, providing therapeutic and educational intervention to promote development and functional participation within a natural environment.
ESSENTIAL FUNCTIONS:
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned.
- Provides direct physical therapy services to Birth to 3 clients in the clients' homes, daycare, and other natural environments using a primary coach approach.
- Completes a plan of care with therapy goals and therapeutic strategies. Sends plan of care and evaluation summary to the child's primary care physician and obtains a plan of care with physician signature for all direct physical therapy services.
- Provides service coordination/case management to families of children found eligible for Birth to 3 services; performs intake visits; requests medical orders needed for evaluations; schedules and attends client evaluations and IFSP meetings; develops transition plans with families; notifies schools in a timely manner; schedules and attends transition meetings; completes billing for service coordination services; maintains client records.
- Provides ongoing monitoring, enhancement, and training of Birth to 3 program requirements.
- Participates in community organizational group as a representative of the Birth to 3 programs and presents for in service and educational purposes on the Birth to 3 Program to community programs as requested.
- Other duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge of Child Development and Sensory Integration Theory.
- Ability to perform detailed work accurately and independently in compliance with stringent time limits with minimal direction and supervision.
- Ability to prioritize duties to accomplish a high volume of work product while adapting to constant changes in priority.
- Ability to maintain accurate and legible notes.
- Knowledge of County policies, procedures, and practices, as well as the organization and departmental operating requirements of local government.
- Ability to establish and maintain effective working relationships with others.
- Ability to analyze and prepare organizational and functional reports.
- General knowledge of standard office procedures, practices, equipment and office assistance techniques.
- Ability to work the allocated hours of the position.
LANGUAGE SKILLS
- Ability to communicate effectively with other members of the staff, supervisor, and the public
- Ability to communicate clearly and concisely in both written and verbal form.
- Must be proficient in workplace English and spelling.
- Ability to read County policies and procedures; written instructions, general correspondence; SDS sheets, safety manuals, maps, etc.

- Ability to perform basic mathematical computations

PHYSICAL REQUIREMENTS
- This work requires the occasional exertion of up to 25 pounds of force; work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to handle, feel, and perform fine motor skills and repetitive motions, and occasionally requires walking, stooping, kneeling, crouching or crawling and reaching with hands and arms.
- Sufficient vision to perform tasks such as reading documents, operating vehicles, or working with detailed data.
- Ability to hear and understand speech at normal levels for tasks such as communicating with the public, colleagues, or over the phone.
- Vocal communication is required for expressing or exchanging ideas by means of the spoken word.
- Work requires preparing and analyzing written or computer data and observing general surroundings and activities.
- Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes, communicable illnesses and diseases and unsafe environments.
- May require dealing with persons who are hostile, aggressive, abusive or violent, posing threatening conditions.
WORK ENVIRONMENT
- Work is performed in an office setting as well as clients' and service providers' homes, schools, daycare centers and work sites.
MINIMUM QUALIFICATIONS:
EDUCATION AND EXPERIENCE REQUIREMENTS
- Licensed Physical Therapist in the State of WI.
- Bachelors, Master’s, or DPT degree from an accredited Physical Therapy program.
- PTAs are not eligible applicants for this position.
- Valid driver’s license or access to transportation to travel between homes across the county.
- Must successfully pass criminal and caregiver background check.
