osition Summary
The PRN Hospice RN provides compassionate, patient-centered care to individuals facing end-of-life. This role supports our interdisciplinary team by performing home visits, completing nursing assessments, managing symptoms, and ensuring patients and families receive exceptional clinical and emotional support.
Key Responsibilities
Deliver skilled nursing care to hospice patients in their homes or facilities.
Complete comprehensive assessments, symptom management plans, and ongoing evaluations.
Communicate changes in patient status to the interdisciplinary team and participate in care planning.
Provide education and support to patients, families, and caregivers.
Perform PRN visits, admissions, recertifications, and triage support as needed.
Maintain accurate, timely clinical documentation in the EMR.
Collaborate with physicians, social workers, chaplains, aides, and other disciplines to ensure coordinated care.
Follow hospice policies, regulatory requirements, and best practices for quality and safety.
Qualifications
Current RN license in the state of practice.
Minimum 1 year of nursing experience; hospice or home health preferred.
Strong assessment and critical-thinking skills.
Ability to work independently in the field and manage changing priorities.
Reliable vehicle, valid driver’s license, and current auto insurance.
Comfortable providing care in a home setting.
Schedule & Expectations
PRN, flexible scheduling based on agency needs.
May include intermittent visits, admissions, on-call backup, or weekend coverage as available.
Mileage or travel reimbursement per agency policy.
