Lead Case Manager bei LEGACY TREATMENT SERVICES GROUP

The Lead Case Manager provides monitoring services and treatment adherence support. Provides screening services as needed. Maintains contact with referring agencies, related people and with community resources used for referral. Maintains coordination with Screening, local and state hospitals, treatment providers, and other treatment team members for consumers. Maintains positive working relationships with community agencies. Demonstrates the knowledge and skills necessary to  provide care appropriate to any age-related needs of the consumers served. Demonstrates the knowledge of the principles of growth and development appropriate to the consumer population served. He or she must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide care based on agency approved evidence based practice models. Represents department at Community and State Meetings. Responsible for overall  coordination of care in the department as well as the supervision of staff. Required to complete annual Agency trainings, as well as job related trainings determined by position. Utilizes SCM, CPR, and  First Aid techniques as required.

The Involuntary Outpatient Commitment (IOC) program offers a new option for individuals who aren't imminently dangerous enough for commitment to an inpatient program, but who struggle with remaining in treatment and are likely to become dangerous without that treatment. IOC coordinates community services and supports that increase the likelihood of treatment adherence. IOC services are a multi-disciplinary approach to ensure that IOC consumers are properly assessed, linked to treatment and services and emotionally supported.

Hours

Position is full time. Hours include flexibility based upon program need.

Education/Experience:

Screener eligibility and MA/MS in mental health or related field with 1 year of experience or BA/BS with 2 years of experience.
