Erie’s Social Drivers of Health (SDOH) Program aims to address the health-related social needs that often act as barriers to patients’ well-being. By focusing on the unique needs and challenges faced by Erie’s patient population, this program seeks to improve health equity and empower individuals to live their healthiest lives.
The Social Drivers of Health (SDOH) Care Coordinators at Erie connect patients with a range of services to address the social drivers of health. This includes linking patients with internal resources at Erie like behavioral health or case management services, as well as connecting patients to local community resources such as rent/utility assistance, food programs, transportation, and legal aid. The SDOH Care Coordinators works closely with patients to understand their unique needs and curate individualized resources to overcome barriers.
The Care Coordinator role is a direct service patient-supporting role, with some administrative responsibilities. The Care Coordinator supports patients in-person in the clinic and works directly with clinical care team members. The Float Care Coordinator ensures consistency of SDOH services across their assigned region by providing on-site support at any of the health center sites in their region as required. The northern region includes: Erie Evanston/ Skokie, Erie HealthReach Waukegan, Erie Foster Ave, and Erie Helping Hands and the southern region: Erie West Town, Erie Division Street, Erie Humboldt Park, Erie Sankofa Village, and Erie School Based Sites. As part of their role, the Care Coordinator screens Erie patients and their families for social driver of health needs including food, transportation, medication assistance, and others. The Care Coordinator also links Erie patients to resources within Erie and in the community. The Care Coordinator enrolls patients in benefits, schedules medical appointments, and navigates social driver of health resources such as rental and utility assistance, food assistance, medical transportation, as well as others.
The Care Coordinator will complete data entry and reporting requirements for SDOH, assist with SDOH patient registry, and assist with managing SDOH resources.

Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
Main Duties & Responsibilities
- Screen patients for needs around SDOH and provide linkage to services such as emergency financial assistance, transportation, food assistance, etc.
- Enroll patients in services, benefits, and programs for which they are eligible (pharmaceutical programs, LINK, financial applications for hospital referrals, etc.)
- Educate and empower patients to navigate future SDOH needs
- Distribute and track SDOH resources including Top Box Food distribution, ordering Uber Health rides, and distributing other material resources
- Use Epic to document work, respond to referrals and create SDOH panels and outreach risks
- Build effective relationships with local social service agencies and advocate for clients in the process.
- Performs other duties as assigned.
Qualifications
Education
- High School Diploma or equivalent required
- Associates degree or Bachelor’s degree preferred
Skills and Knowledge
- A minimum of two years of work experience in community health, social or community services or comparable experience preferred
- Bilingual fluency (English/Spanish) strongly preferred
- Local travel required
- Intermediate proficiency in MS Office preferred
- Verbal and written communication skills, including high level of comfort presenting to groups
