Position Purpose

We are looking for a detail oriented, compassionate, adaptable, outside of the box thinker and person-centered care focused LPN to fill the role of the Nursing Care Coordinator (NCC)! The NCC will ensure that admissions, audits, assessments, and resident care meetings are conducted accurately, timely, and with the resident and their care as the priority.  The Nursing Care Coordinator will preferably have experience with Functional Capacity Screenings (FCS) and Negotiated Service Agreements (NSA), conducting resident care meetings and be comfortable in the advocate role. While partnering with the Assisted Living Operator and Resident Care Coordinator, the NCC will have a key role in assessing resident needs, meeting with residents and their families/POA’s to determine appropriate support service teams and other interdisciplinary team members to ensure residents are receiving person centered care and their nursing needs are met.

Key Responsibilities/Duties

Point Click Care Nursing Audits: Including the following, but not limited to:

  • monthly skin assessments
  • administration EMAR signoffs
  • medical management pending adjustments for medications (blood pressure, insulin, etc.)
  • physician orders
  • progress notes
  • Fall packets
  • Back up for admission audits
  • Additional audits could be added contingent upon needs of department
  • Point of Care completion audits

Management, Organization and Completion of Assessments and Screenings including the following, but not limited to:

  • Monthly Skin Assessments
  • FCS quarterly and with any significant change
  • NSA yearly and with any significant change
  • BIMS quarterly
  • Depression quarterly

Assessing, obtaining orders and performing treatments and follow up with practitioners as needed for skin integrity concerns and wound management.

Performs duties in conjunction with operator and physicians to collaborate and coordinate clinical quality improvement with activities within numerous internal departments, clinical staff and families.

Responsible for management of physician orders from campus medical director and community physicians as required in PCC.

Ability to self-direct, make independent decisions and prioritize daily tasks.

Ability to be flexible and adapt, to change in a timely manner.

Will be part of the on-call rotation

Qualifications


Qualifications:

  • Current unrestricted KS LPN licensure required
  • Minimum of 1-2 years of LPN experience
  • Wound Certification and/or Wound Experience (Preferred)
  • Experience with auditing and management of orders
  • Experience with Functional Capacity Screenings (FCS) and Negotiated Service Agreements (NSA)
