GENERAL STATEMENT OF RESPONSIBILITIES: 

Under the direct supervision of the Nursing Supervisor, screens patients and prepares for exams by Providers. Administers medications/immunizations under the direction of the Providers. Duties include meeting with patients and their families, connecting patients with professional services, and maintaining consistent communication with patients to assess their wellbeing. The LVN prepares and assists health care providers, as needed with exams. Maintains professional knowledge of LVN duties and responsibilities. Substitutes at Campo Medical clinic on an as needed basis. 

SPECIFIC DUTIES AND RESPONSIBILITIES:

  1. Assessment of the patient’s physical, mental wellness, needs, preferences, financial status, and abilities. The RNCM uses this information to develop individualized care plans. 
  2. Prepare patients for medical examinations, ensuring comfort and readiness.
  3. Works collaboratively with patients, families, medical providers, nurses, Tribal entities, and all members of the multidisciplinary healthcare team to provide high quality and patient-driven care.
  4. Ensure accurate and timely documentation in the Electronic Health Record (EHR) system.
  5. Provide patient education within scope of practice, reinforcing provider instructions and promoting health literacy.
  6. Recognize and respond to patients’ emotional and psychological needs in a supportive and professional manner.
  7. Documents patient progress in NextGen electronic health record (EHR) related to the plan of care, including charting referrals, home visits and other notable interactions.
  8. Documents intake information, reassessment information, referrals, case reports and home visits within the EHR.
  9. Schedule referrals and follow-up appointments, ensuring patients understand instructions, dates, and transportation options.
  10. Coordinate transportation services for patients when appropriate.
  11. Maintain follow-up files and proactively notify patients of upcoming or overdue care.
  12. Determines the need for medical, therapeutic, psycho-social, or psychiatric evaluations based on patient progress. May initiate referrals based upon IHC standardized protocols in collaboration with the medical team.
  13. Respond to patient phone inquiries or route calls appropriately to other staff.
  14. Coordinates and provides patient care based on an approved treatment plan.
  15. Ensures that patients are active participants and decision makers in their treatment plan, answering any questions they may have.
  16. Builds strong relationships with various health care discipline teams throughout the case management process.
  17. Participates in staff meetings, continuing education, and quality improvement initiatives.
  18. Attend trainings and professional development conferences as requested.
  19. Complies with all SIHC policies & procedures. 
  20. Ability to work flexible schedule may be required based on need.
  21. Other duties as assigned.


QUALIFICATIONS:

Education/Experience:    Must possess a current California Licensed Vocational Nurse license. Must have minimum of one-year experience in an ambulatory care environment. Prior case management or care coordinator experience with complex circumstances preferred. 3 years of relevant medical experience preferred. Must be at least 18 years of age. Must have knowledge and understanding of HIPAA in the ambulatory care setting, with absolute ability to maintain confidentiality according to standards.  Must possess current BLS certification.

Licenses/Certifications:   A valid driver’s license is required and must be maintained throughout employment. Applicant must be able to travel throughout the organization’s service area including all SIHC site locations. Current certifications and/or licenses appropriate to the positions required education and profession.

Character:   Applicants must have a reputation for honesty and trustworthiness.  Must be responsible and able to exercise good judgment, accept administrative supervision, pay attention to detail, follow instructions, including the ability to interact effectively and communicate with people in a professional and courteous manner.  Must be highly confidential and work as a team with other staff.  Applicants should be sensitive to client’s needs.

Ability to establish and maintain effective peer relationships with coworkers within the Department, clinic-wide, and the public. Physical and Personal Requirements: Normal clinic/office environment. Sit or stand for long periods of time. Reach, bend, climb, stoop, lift up to 25 lbs. repetitive hand movement; use and view PC. Reliable transportation and car insurance as required by the state. Travel as needed.

Compliance: Each employee plays a key role in creating a compliance culture at SIHC. Employees are expected to learn and comply with all SIHC policies and procedures. One must have the ability to understand the implications and complexities of all compliance policies. Participation in all compliance training is mandatory, and generally has a heightened awareness of policies and regulations that are specific to one’s department and position. All SIHC employees are expected to report violations or suspected violations of compliance policies or regulations. 

OtherApplicants must successfully pass a pre-screening tuberculin skin test or x-ray and a blood/urine drug screening test.  Health must be adequate to perform all duties of the position.  Applicants must complete SIHC Application and Authorization Form, both must be submitted to Human Resources prior to the close date indicated.

INDIAN PREFERENCE

INDIAN PREFERENCE: In the filling of any SIHC job vacancy, preference may be given to qualified Native American Indians, pursuant to the Indian Preference Hiring Act, 25 USC 472, unless other laws require the filling of a vacancy without regard to Indian preference (e.g.   Public Law (P.L.) 94-437, “Indian Health Care Improvement Act (IHCIA).   To receive Indian Tribe ore for any SIHC position, the applicant must be enrolled, or be eligible for enrollment, as an American Indian with their Tribe, or must be certified as an American Indian from the designated Tribal Representative. Applicants claiming Indian preference must attach verification of their claim to the SIHC application, including Certification of form BIA-4432, which is available from the SIHC Human Resources Department. If verification is not or cannot be verified, the applicant will not receive Indian preference for purposes of the interview.
